Renew the ambulance permit issued to Southern Trinity Health Services.

RECOMMENDATION: Receive and file the new
petition, affidavit of valuation and engineer's report for the formation of
Permanent Road Division No. 1000, Zone No. 002.
Adopt the Resolution of Intent to Establish Humboldt County Permanent
Road Division No. 1000, Zone No. 002, to Impose Parcel Charges for Road
Maintenance and to Rescind Resolution No. 04-72. Set a time and date of on
Code § 53753. Direct the Department of Public Works to post the petition, notice of formation and notice of assessment as per Streets and Highway Code
Various County Departments--Hiring Freeze Exemptions
RECOMMENDATION: Approve an exemption for an Office Assistant I/II in CDS; a Deputy DA II, a Deputy DA IV; a Senior Library Assistant (Garberville Branch); and a Correctional Work Crew Leader.

4. Standing Item--Adjourn as County Board of Supervisors, and Reconvene as Humboldt County Public Authority. Pursuant to Government Code Section 54957.6, Conference with Labor Negotiator. It is the intention of the Board of the Humboldt County Public Authority to meet in Closed Session to have a conference with its Labor Negotiator to review and instruct designated representatives with respect to the various forms of compensation provided to represented and unrepresented personnel of the Humboldt County Public Authority.

RECOMMENDATION: Adopt the proposed Negative Declaration as required by Section 15074(b) of the CEQA Guidelines. Deny the appeal, and approve the Conditional Use Permit, Surface Mining Permit, Reclamation Plan and Financial Assurance Cost Estimate subject to the recommended conditions of approval.
