Board of Supervisors
RECOMMENDATION: Ratify the Resolution.
RECOMMENDATION: Authorize Chair to sign letter of appreciation.
RECOMMENDATION: Ratify the Proclamation.
RECOMMENDATION: Authorize Chair to sign Letter of Thanks and Certificate of Recognition.
RECOMMENDATION: Declare that a local state of emergency, Per Resolution #07-87 remains in effect until May 20, 2008.
Clerk of the Board
RECOMMENDATION: Approve the Board Order, and authorize the Clerk of the Board and the Board Chair, to sign the minute book in accordance with California Government Code Section 25103.
County Administrative Office
RECOMMENDATION: That the Board of Supervisors approve the request from the Communications Division of the County Administrative Office to declare the supply of used cell phones and accessories as surplus and have the Purchasing Agent dispose of them.
RECOMMENDATION: That the Board of Supervisors adopt the forty-hour class of Senior IT Systems Administrator, salary range 341 (class 0126, unit 1) into the classification plan, and allocate on full-time position in budget unit 118 (salary range 341, class 0126) effective upon approval.
RECOMMENDATION: That the Board of Supervisors approve an overlap of 2-1/2 weeks (May 14 - June 2, 2008) for the position of Deputy County Counsel IV (salary range 489, class 0600, pos #06), effective May 14, 2008, pursuant to Section 7 of the Salary Resolution (4/5 vote required).
Health and Human Services - Mental Health Branch
RECOMMENDATION: It is recommended that the Board of Supervisors approve the allocation of two full-time Psychiatric Mid Level Practitioners (salary range 450, class 0912) in budget unit 424 effective the beginning of the biweekly pay period following approval.
RECOMMENDATION: That the Board of Supervisors approve and authorize the Chair to sign the agreement with Particular Council of the Redwood Region of the Society of St. Vincent de Paul for distribution and administration of Mental Health Services Act (MHSA) Housing Set aside funds for MHSA Full Service Partners and/or clients of MHSA programs.
Health and Human Services - Public Health Branch
RECOMMENDATION: That the Board of Supervisors approve Amendment A01 to Agreement Number 07-65050 with the California Department of Public Health; Authorize the chair to sign the first page of the standard agreement amendment packages; Authorize the Public Health Branch Director to sign three copies of the first page of amendment A01 to the HIV Prevention Program Memorandum of Understanding and three copies of the first page of amendment A01 to the Early Intervention Program Memorandum of Understanding; Approve a budget revision in Fund 1175, Budget Unit #402, Budget Unit #437 and Budget Unit448; Approve the new Budget Unit #449 in Fund 1175 and direct the Auditor-Controller to create the budget.
13. Agreement with the California Department of Health Services for the California Violent Death Reporting System and approve the supplemental budget for Fund 1175, Budget Unit 400, Fiscal Year 2007-08.7
RECOMMENDATION: Approve and authorize the Public Health Branch Director to sign the agreement with the California Department of Health Services for the California Violent Death Reporting System and approve the supplemental budget for Fund 1175, Budget Unit 400, Fiscal Year 2007-08.
RECOMMENDATION: That the Board of Supervisors approve the Agreement between California State University at Northridge and the Department of Health and Human Services, Public Health Branch, Medical Therapy Unit for an intern Physical Therapy Program for the period of May 19, 2008 through May 19, 2011 or three years.
RECOMMENDATION: That the Board of Supervisors approve and authorize the Public Health Branch Director to sign the Memorandum of Understanding between the Department of Health and Human Services, Public Health Branch and North Coast Emergency medical Services for the distribution of Pediatric Richie fund; approve the supplemental budget and direct the Auditor/Controller to adjust the budget in fund 1175, budget Until #410, Emergency Medical Services.
RECOMMENDATION: That the Board of Supervisors authorize the Chairperson of the Humboldt County Board of Supervisors to sign three originals of Public Health Emergency Preparedness Centers for Disease Control and Prevention Agreement for funding period of August 31, 2007 through August 9, 2008.
Health and Human Services - Social Services Branch
RECOMMENDATION: That the Board of Supervisors approve lease with Christopher and Marie Wilkins for office space for the Public Guardian at 23 Fifth Street and authorize the Chairperson to execute the originals of the lease.
18. Memorandum of Agreement with Access Humboldt for the provision of Community Broadband and wireless networking services to the public at the Eureka Main Library and the Rio Dell Branch Library in the City of Rio Dell.
RECOMMENDATION: That the Board of Supervisors approve the Memorandum of Agreement with Access Humboldt; Accept the non-cash gift donation of Access Humboldt Services as detailed in the Memorandum of Agreement and authorize Victor Zazueta, Director of Library Services to sign three copies of the Memorandum of Agreement.
RECOMMENDATION: That the Board of Supervisors approve the temporary closure of the Rio Dell branch Library in the community of Rio Dell for remodeling.
RECOMMENDATION: That the Board of Supervisors adopt Fiscal Year 2008-2009 Northern California Regional Facility daily detention bed rate in the amount of $171.89 for all referring counties.
RECOMMENDATION: That the Board of Supervisors extend Laval Lau's extra help employment by 300 hours to a maximum of 1260 hours for Fiscal Year 2007/2008, pursuant to Section 7 of the Humboldt County Salary Resolution.
RECOMMENDATION: That the Board of Supervisors authorize the Chief Probation officer to sign the agreement with Shasta County for the purpose of accessing Assessments.com licenses and participating in the training and implementation of Assessments.com risk/needs tools through Shasta County.
RECOMMENDATION: That the Board of Supervisors approve the extension of extra help hours for Cathy Dreyfuss, Deputy Public Defender, by 500 hours to a maximum of 1460 hours for Fiscal Year 2007/2008, pursuant to Section 7 of the Humboldt County Salary Resolution.
RECOMMENDATION: That the Board of Supervisors find that the License with the Alderpoint Volunteer Fire Department is in the public interest in that the parcel will be used for the storage of fire fighting equipment for the residents of the Alderpoint area; find that the interest of land to be used by the Alderpoint Volunteer Fire Department pursuant to the terms of the License will not substantially conflict or interfere with the use of the property by County; Approve and authorize the chairperson to execute the License Agreement in duplicate with the Alderpoint Volunteer Fire Department.
RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department, Between Granite Construction Co., Contractor, and Humboldt County for the asphalt base and rehabilitation on McKinleyville Avenue from the High School to Hiller Road.
RECOMMENDATION: That the Board of Supervisors approve the following supplemental budget to provide finds for the Dowd Road portion of the Martin's Ferry Project: Revenue: 1200-325-523-172 - $2,750,000; Expenditures: 1200-325-2-155 - $2,750,000.
RECOMMENDATION: That the Board of Supervisors authorize the Chair to sign three copies of Agreement No. X08-5904 (093).
RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department between DANCO, Contractor, and Humboldt County for the construction of sidewalks on Central Ave. and Murray Rd; Approve the final claim for payment as submitted by the Public Works Department to the Auditor Controller, in the amount of $21,824.48. This will bring the total amount paid to 100% of the contract.
RECOMMENDATION: That the Board of Supervisors approve a one year extension of the lease with Charlene J. Lundblade and Ron B. Lundblade for property located at 112 W Third Street, Eureka, CA; Authorize Real Property to send a notice to the Lessor that County is exercising the option to extend the lease.
RECOMMENDATION: That the Board of Supervisors approve an increase in extra help hours for Correctional Officer Lori Marriott from 960 Hours to 1260 Hours pursuant to Section 7 of the Humboldt County Salary Resolution.

RECOMMENDATION: Open the public hearing regarding the formation of Permanent Road Division No. 1000, Zone No. 005 and the setting of a special parcel charge for maintenance of roads for Hidden Meadow Subdivision, receive public testimony and continue the Public Hearing and Election to June 10, 2008 at 1:45 p.m.
