Agricultural Commissioner

1.                   Adoption of Ordinance Amending Humboldt County Code Sections 715-3 and 715-6 Related to the County Commercial Weighing and Measuring Device Registration Program.

RECOMMENDATION: That the Board of Supervisors adopt Ordinance No. ___, Amending Chapter 5 of Division 1 Title VII of the Humboldt County Code relating to the County's commercial weights and measures device registration program, and that within 15 days after the adoption of the ordinance, the Board shall publish a summary of the ordinance and the Clerk shall post in the office of the of the Clerk of the Board a certified copy of the full text of the adopted ordinance and amendments along with the names of those Supervisors voting for and against the ordinance.  Government Code Section 25124(b)(1).

4.                   Letter of Appreciation to Karen Lofts Jarbo for Serving on the Workforce Investment Board (WIB) Youth Council. (Supervisor Smith)

RECOMMENDATION: Authorize Chair to sign Letter of Appreciation.

5.                   Letter of Appreciation to Steve Launi for Serving on the Forestry Review Committee. (Supervisor Smith)

RECOMMENDATION: Authorize Chair to sign Letter of Appreciation.

6.                   Loleta Market's Application to Alcohol and Beverage Control for Offsale 21 Hard Liquor License. (Supervisor Smith)

RECOMMENDATION: Determine the issuance of an Offsale 21 Hard Liquor License for Loleta Market does meet public convenience criteria and authorize Chair to sign letter of determination to Alcohol and Beverage Control.

7.                   Appointment to the Humboldt Harbor Recreation & Conservation District's, Humboldt Bay Master Plan Advisory Committee (Supervisor Smith)

RECOMMENDATION: Approve the appointment of Donald C. Tuttle, as the County's representative on the Humboldt Bay Master Plan Advisory Committee.

8.                   Appoint Erica Roane to the Orick Design Control Committee. (Supervisor Geist)

RECOMMENDATION: Approve the appointment.

Community Development Services

9.                   Miller Joint Timber Management Plan Guide File No. APN 316-063-03 et al.  Case No: JTMP-07-02 Willow Creek Area

RECOMMENDATION: That the Board of Supervisors approve the Miller Joint Management Plan (JTMP) and Guide Pursuant to Section 51119.5 of the California Government Code (C.G.C) (Attachment A); Direct the Community Development Services - Planning Division to record a Declaration of Covenants, Conditions and Restrictions Implementing the JTMP and Guide with the JTMP (Attachment B); Direct the Clerk of the Board to give notice of the decision to Steve Miller, the County Assessor's Office and any other interested party.

10.               Request for Extension of Extra Help Hours for Budget Unit 114.

RECOMMENDATION: That the Board of Supervisors approve the extension of extra help hours for Pamela Sloan, Revenue Recovery Officer I, from 960 hours to 1984 hours through June 30, 2009, per Section 7 of the Humboldt County Salary Resolution.

11.               Adoption of Annual Resolutions Authorizing New Chair and Vice-Chair of the Board of Supervisors to take all Acts Necessary to Act as Chair and Vice-Chair, Including Authorized Signatures on Necessary Documents of Financial Transactions.

RECOMMENDATION: That the Board of Supervisors adopt the attached three resolutions authorizing the new Chair and Vice Chair of the Board of Supervisors to take all acts necessary to act as Chair and Vice-Chair, including authorize signatures on necessary documents of financial transactions, for acceptance and consent to grants of real property and authorize the Chair to sign said resolutions.

District Attorney

12.               Donation of District Attorney Department Vehicle to the County Motor Pool for Capitalization, Maintenance, and Insurance.

RECOMMENDATION: That the Board of Supervisors approve the donation of one District Attorney's Investigate Unit 2008 Vehicle (No. 04546) to the County Motor Pool for capitalization, maintenance, and insurance.

Health and Human Services - Mental Health Branch

13.               Request for Extension of Extra-Help Hours for Budget unit 1170-424 Mental Health Branch Administration.

RECOMMENDATION: That the Board of Supervisors approve the extensions of extra-help hours for budget Unit 1170-424 pursuant to section 7 of Humboldt County Resolution (4/5 vote required) effective immediately as follows: Clare Adams, Children's Center Specialist to a maximum of 1460 hours; Genevieve Carlson, Activity Therapist to a maximum of 1260 hours; Shayna Clark, Office Assistant I to a maximum of 1460 hours; Jeanne Franklin, Medical Office Assistant to a maximum of 1460 hours; Alana Sieck, Activity Therapist to a maximum of 1460 hours and Christoper Sweitzer, Psychiatric Nurse to a maximum of 1460 hours.

14.               Temporary Salary Increase in Accordance with Section 7 of the Humboldt County Salary Resolution.

RECOMMENDATION: It is recommended that the Board of Supervisors authorize temporary increase in compensation for Senior Case Manager, Pamlyn Millsap, of 10 salary ranges, beginning January 11, 2009 and continuing until classification studies have been completed and positions in the Rose Program have been filled, pursuant to Section 7 of the Humboldt County Salary Resolution.

Health and Human Services - Public Health Branch

15.               Approval of Ambulance Permits for Fiscal Year 2008-2009.

RECOMMENDATION: It is recommended that the Board of Supervisors authorize the health Officer to renew existing ambulance permits for the following:  Service Area I: Arcata-Mad River Ambulance Service; Services Area II:  Hoopa Health Association; Service Area III: City Ambulance: Service Area IV: City Ambulance Fortuna/Southern Trinity Area Rescue; Service Area V: City Ambulance Garberville; n/a: Cal-Oregon Air Flight.

Health and Human Services - Social Services Branch

16.               Disallocate Eight (8) Vacant Social Worker I/II/III Positions; Allocate Eight (8) Social Worker IV Positions, upon vacancy, to Social Worker IV Positions in Child Welfare Services and Adult Protective Services.

RECOMMENDATION: That the Board of Supervisors approve disallocation of: Eight full-time Vacant Social Worker I/II/III positions (salary range 323/348/364, class 072, positions #05,06,07,11,15,16,18,20 in Budget Unit 511); Approve allocation of: Eight full-time Social Worker IV positions (salary range 374/388/390/420, class 0707 in Budget Unit 511); and Approve the reallocation of future vacant Social Worker I/II/III positions to Social Worker IV-A/B/C/D when vacated in Child Welfare and Adult Protective Services Programs effective upon approval.

17.               Disallocate Vacant Full-Time Administrative Secretary and Allocate one Full-Time SSB Secretary II (Merit System Services classification) in Budget Unit #511, Social Services Branch.

RECOMMENDATION: That the Board of Supervisors approve the disallocation of one vacant full-time Administrative Secretary (class 0166, salary rang 331, position #01) and allocate one SSB Secretary II (salary range 331, class 331) position in budget unit 511 effective immediately.

18.               Approval of Software License Purchase and Annual Maintenance Agreement with Biready Incorporated.

RECOMMENDATION: That the Board of Supervisors approve the software license purchase; and Accept conditions of the annual maintenance agreement with BIReady Incorporated; and Authorize the Chair or Vice Chair to sign three (3) copies of the Agreement; and Direct the Clerk of the Board to return two copies of the Agreement to the Social Services branch; and Approve the attached Appropriation Transfer from Fund 1160, line item 511-2-148 to Fund 1160, fixed asset line item 511-8-965.

19.               Allocate One Full - Time Vocational Assistant Position in Budget Unit 511 (Social Services)

RECOMMENDATION: It is recommended that the Board of Supervisors approve allocation of one 1.0 FTE Vocational Assistant (class 0742, range 248) position in budget unit 511 (Social Services) effective upon approval.

20.               Approval of First Amendment with Alcohol Drug Care Services, Inc. to Expand the Operation of a Winter Shelter.

RECOMMENDATION: That the Board of Supervisors approve the First Amendment with Alcohol Drug Care services, Inc. to expand the operation of a winter shelter; Authorize the Chair to execute three copies of the Amendment; and Direct the Clerk of the Board to route two fully executed copies of the Amendment to the Social Services Branch Director.

RECOMMENDATION: It is recommended that the Board of Supervisors authorize temporary increase in compensation for Program Coordinator, Bruce Alexander, of 10 salary ranges, beginning January 11, 2009 and continuing until a classification study has been completed and the position has been filled, pursuant to Section 7 of the Humboldt County Salary Resolution.

22.               Approve Memorandum of Understanding with Community Health Alliance of Humboldt-Del Norte, Inc. to ensure Health Care Coverage to Children living in Humboldt County.

RECOMMENDATION: That the Board of Supervisors approve Memorandum of Understanding  with Community Health Alliance of Humboldt – Del Norte, Inc., to collaborate in providing health care coverage to children living in Humboldt County through Medi-Cal, healthy Families, and Healthy Kids Humboldt Program; Authorize the Chair to execute three copies of the memorandum of Understanding; and Direct Clerk of the Board to route two fully executed copies of the Memorandum of Understanding to the Director, Department of Health and Human Services Social Services Branch.

Library Services

23.               Literacy Services Agreement with Humboldt Literacy Project and Budget Revision in Fund 1500, Budget Unit 621, County Library

RECOMMENDATION: That the Board of Supervisors approve the attached Literacy Services Agreement with Humboldt Literacy Project for providing literacy training for adults in Humboldt County; Authorize the Chairperson to sign the attached Literacy Services Agreement and two (2) additional copies of the signature page; Direct the Clerk of the Board to return the two (2) signed pages of the Literacy Services Agreement to the Library; Adopt a budget revision in Fund 1500, Budget Unit 621, County Library and authorize the Auditor-Controller to revise the budget as shown in Attachment 1.

24.               Adjustment to Library Bookmobile Schedule.

RECOMMENDATION: That the Board of Supervisors adopt a revised winter schedule for the Library Bookmobile as shown on Attachment 1.

Personnel

25.               Revised Class Description for Chief Appraiser.

RECOMMENDATION: That the Board of Supervisors approve the revised class description for Chief Appraiser.

Public Works

26.               Construction of Rohnerville Airport Phase II Runway and Taxiway Lighting Project: Project No.:  AIP 3-06-0083-07, Contract No. 919161.

RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department, between Danielson Construction, Inc., Contractor, and Humboldt County for the construction of the Rohnerville Airport Phase II Runway and Taxiway Lighting Project.

27.               Adoption of Ordinance Designating Various Speed Zones on Briceland Thorne Road and Shelter Cove Road

RECOMMENDATION: That the Board of Supervisors adopt Ordinance No. ___ amending Sections 422-7, 422-10, and 422-12 of Title IV of the Humboldt County Code relating to speed limits on Briceland Thorne Road and Shelter Cove Road in Whitethorn and Shelter Cove.

28.               Construction of Dinsmore Airport Runway 9/27 Rehabilitation: Project No.: AIP 3-06-0361-05, Contract No. 917221.

RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders previously approved by the Board of Supervisors, between Granite Construction Company, Contractor and Humboldt County for the construction of the Dinsmore Airport Runway 9/27 Rehabilitation.

29.               Construction of Storm Damage Repairs on Cathey Road (6D050) at PM 0.47 and 0.70: Project No.: FEMA CA DR 1628, HC 58 & HC 60, Contract No. 206285 & 206327.

RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department between Granite Construction, Contractor, and Humboldt County for the storm damage repairs on Cathey Road; and Approve the final claim for payment, as submitted by the Public Works Department to the Auditor/Controller, in the amount of $23,421.40.  This will bring the total amount paid to 100% of the contract.

30.               Monument Preservation Fund Update and Supplemental Budget Request.

RECOMMENDATION: That the Board of Supervisors receive the annual update on expenditures from the Monument Preservation fund; Approve the following supplemental budget to provide funding for ongoing projects: Revenue: 1200322-705800 $18,581; Expense:  1200322-2118  $18,581.

31.               Transfer from Road Fund Contingencies for Trench Work on Ishi Pishi Road. ($85,272)

RECOMMENDATION: That the Board of Supervisors approve the attached $85,272 transfer from Road Fund Contingencies.

32.               Surplus Parks Division Equipment - 1955 John Deere Tractor with Brush Mower Attachment and 1987 Travel Trailer.

RECOMMENDATION: That the Board of Supervisors declare a 1955 John Deere tractor with Brush Mower attachment and a 1987 travel trailer within the Parks Division as surplus equipment; Authorize the Humboldt County Purchasing Agent to sell the two identified surplus items; Deposit revenue from sales into the miscellaneous revenue account of the Parks Division Fund (1100713-707010).

33.               Construction of Grassy Creek Culvert Replacement on Fieldbrook Road (C3M0S0) at PM 2.16: Agreement No. P0510307; Contract No. 206140.

RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department, between miller Farms, Contractor and Humboldt County of the replacement of Grassy Creek Culvert; approve the final claim for payment, as submitted by the Public Works Department to the Auditor/Controller, in the amount of $62,950.68. This will bring the total amount paid to 100% of the contract.

34.               Construction of Storm Damage Repairs on Alderpoint Road (F6B165) at PM 19.95: Project No.: ER 4400(041), Contract No. 206353.

RECOMMENDATION: That the Board of Supervisors authorize the Director of Public Works to sign Contract Change Order number 1 for the total amount of $66,053.24; approve the following Supplemental Budget: Revenue 1200-321-523190 $56,014.00;  Expenditures 1200-321-0230 $56,014.00; approve the attached Appropriation Transfer to provide for the Local Share.

35.               Construction of Storm Damage Repairs on Trinidad Scenic Drive (4M310) at PM 1.14: Project No. FEMA C DR 1628 HC13, Contract No. 206287.

RECOMMENDATION: That the Board of Supervisors accept the completed contract, including all change orders approved by the Public Works Department, between Van meter Logging, Contractor, and Humboldt County for the storm damage repairs on Trinidad Scenic Drive; Approve the final claim for payment, as submitted by the Public Works Department to the Auditor/Controller, in the amount of $30,035.00.  This will bring the total amount paid to 100% of the contract.

36.               Extension of Extra-Help Hours for Craig Viera, Maria Herrera and Teresa Miranda.

RECOMMENDATION: That the board of Supervisors retroactively extend the extra-help employment hours of Craig Viera, Maria Hererra and Teresa Miranda by 1000 hours to a maximum of 1960 hours for fiscal year 2008-2009 pursuant to Section 7 of the Humboldt County Salary Resolution.

1.                   Report on the Concept of Adopting and Ordinance for the use of Local Option Real Estate Transfer Disclosure Statements in Unincorporated Areas.

RECOMMENDATION: Receive Staff report; receive public testimony and deliberate; Direct staff to conduct additional research, work with interested parties and prepare a Local Real Estate Transfer Disclosure Ordinance for review by the Planning Commission and adoption by the Board of Supervisors in conjunction with the adoption of the General Plan Update.
