These matters are routine in nature and are usually approved by a single vote.


1.                   Employment Agreement with Loretta Nickolaus, County Administrative Officer

RECOMMENDATION: Approve employment agreement through December 20, 2009.

Health and Human Services - Administration

2.                   Appointments to the In-Home Supportive Services Advisory Committee

RECOMMENDATION: Appoint Andrew Buschkamp and Madeline Lawson to said committee.

Health and Human Services - Mental Health Branch

3.                   Services Agreement with Remi Vista, Inc. (Eureka and Redding) for the Provision of Mental Health, Case Management, and Crisis Intervention Services to Medi-Cal Beneficiaries for Fiscal Years 2004 and 2005 ($637,360 Per Fiscal Year for Both Eureka and Redding Sites)

RECOMMENDATION: Authorize Chair to sign said agreement.

4.                   Early Years Program--Humboldt First Five Grant

RECOMMENDATION: Accept grant ($10,000 per year for three years) to provide funds for the services of a Mental Health Case Manager to serve the needs of families with children from birth to age five years.

Health and Human Services - Public Health Branch

5.                   Request for Approval of Supplemental Budget and Amendment to Agreement with California Department of Health Services for Childhood Lead Poisoning Program; Fund 1175, Fiscal Year 2004-05

RECOMMENDATION: Approve supplemental budget ($800) and amendment to agreement, which increases the maximum amount payable from the State by $800 through June 30, 2005.

6.                   Purchase of a Laser Printer for the Waste Tire Enforcement Grant Program

RECOMMENDATION: Approve purchase ($2,400).

Health and Human Services - Social Services Branch

7.                   Agreement with New Directions Foster Family Association

RECOMMENDATION: Approve agreement for $9,415 for each fiscal year (2004 and 2005) for the recruitment and training of prospective foster care providers.

Library Services

8.                   Annual Contract with the Humboldt Literacy Project

RECOMMENDATION: Approve contract for providing literacy training for adults in Humboldt County, and adopt supplemental budget ($10,052).

9.                   Recognition of Donations Made to the Library and Supplemental Budget ($2,228)

RECOMMENDATION: Adopt supplemental budget for receiving donations from the Friends of the Arcata Library, the Friends of the Fortuna Library, and from several individuals.

County Administrative Office

RECOMMENDATION: Receive update, and take appropriate action (if required).

RECOMMENDATION: Take appropriate action (if required).

2.                   Standing Item--Pursuant to Government Code Section 54957.6, Review the County's Position, and Instruct Designated Representatives with Respect to the Various Forms of Compensation Provided to the County's Represented and Unrepresented Employees   (if required)

3.                   Standing Item--Report on Pending or Threatened Litigation Pursuant to Government Code Section 54956.9   (if required)

4.                   Standing Item--Adjourn as County Board of Supervisors, and Reconvene as Humboldt County Public Authority.  Pursuant to Government Code Section 54957.6, Conference with Labor Negotiator.  It is the intention of the Board of the Humboldt County Public Authority to meet in Closed Session to have a conference with its Labor Negotiator to review and instruct designated representatives with respect to the various forms of compensation provided to represented and unrepresented personnel of the Humboldt County Public Authority.

G.                 PUBLIC HEARING--1:30 P.M.

Public Works

1.                   Proposed General Vacation of a Public Right of Way Known as Old Highway 36; Swains Flat Area

RECOMMENDATION: Adopt Resolution No. 05-__, vacating that portion of Old Highway 36 as described in said resolution.

H.                  PUBLIC APPEARANCES           (limit of three minutes per speaker)

This portion of the meeting is reserved for persons desiring to address the Board on any matter not on agenda that is under the jurisdiction of the Board.  It is requested that speakers voluntarily sign the sheet at the front doors of the Chamber.

I.                     ADJOURNMENT

Persons wishing to file documentation on any agenda item for the official record must submit an original and nine (9) copies of each document to the Clerk of the Board of Supervisors, Room 111 of the County Courthouse, 825 5th Street in Eureka (476-2384). Documentation includes, but is not limited to, written correspondence, audio and video tapes, maps, photographs, and petitions. Failure to submit the required number of copies will result in the document/s not being placed in the official record. (Per Board of Supervisors' policy adopted on March 21, 1995).
