14.               Position Allocation

RECOMMENDATION: Reallocate the Administrative Analyst I/II position, currently occupied by Ellen Hook in the Revenue Recovery Division, to Program Coordinator.

15.               County and Courts Reimbursement - Settlement Agreement and Plan

RECOMMENDATION: Authorize CAO to complete settlement negotiations as detailed in Memorandum of Understanding dated March 24, 2005.

16.               Temporary Increase in Compensation for Facility Maintenance Supervisor, Senior Buyer, and Systems Support Technician II

RECOMMENDATION: Approve the continuation of the temporary increase in compensation for Facility Maintenance Supervisor, Craig Wilson, of ten (10) salary ranges beginning March 12, 2005 and continuing until the reorganization of the Building Maintenance Division is complete, or September 12, 2005, whichever occurs first.  Approve the continuation of the temporary increase in compensation for Senior Buyer, Jackie Shema, of ten (10) salary ranges beginning April 13, 2005; and continuing until the new Purchasing Agent is hired, or continued reorganization of the Purchasing Division is complete, or October 13, 2005, whichever occurs first.  Approve the continuation of the temporary increase in compensation for System Support Technician II, Ron Vetter, of ten 10) salary ranges beginning March 29, 2005, and continuing until the reorganization

of the Information Services Division is complete, or September 29, 2005, whichever occurs first.

17.               Temporary Increase in Compensation for Computer Operation and Production Supervisor and Senior Systems Programmer Analyst

RECOMMENDATION: Approve the continued temporary increase in compensation for Computer Operation and Production Supervisor, Norman Reed, of twenty (20) salary ranges beginning April 25, 2005 and continuing until the reorganization of the Information Services Division is complete or October 25, 2005, whichever occurs first.  Approve the continued temporary increase in compensation for Senior Systems Programmer Analyst, Sharon Hiney, of ten (10) salary ranges beginning April 25, 2005 and continuing until the

reorganization of the Information Services Division is complete, or October 25, 2005, whichever occurs first.

18.               Supplemental Budget

RECOMMENDATION: Approve a supplemental budget in the amount of $40,300.00.  Transfer $40,300.00 from the Radio Capitalization Fund 4490-000-7000 to 1100151 -707140 Communications Trust.  Increase revenue estimate in revenue account 1100151 -707140 by $40,300.00.  Increase Communications line 1100151-2112 maintenance of equipment by $30,000.  Increase Communications line 1100151-8692 fixed asset, radio by $10,300.00.  Authorize General Services Department to purchase 16 radios for the Probation Department.  Authorize General Services Department to purchase one radio for Public Works Department.

Health and Human Services - Administration

19.               Request to Reallocate the Senior Health Program Manager Position in the Public Health Branch to Program Manager (Unlicensed)

RECOMMENDATION: Approve request.

20.               Allocate One Sr. Health Education Specialist Position and One Health Education Specialist I/II Position in Budget Unit 414; Allocate One Public Health Nurse Position in Budget Unit 460

RECOMMENDATION: Approve the allocation of one full time Sr. Health Education Specialist.  Approve the allocation of one .5 FTE Health Education Specialist I/II.  Approve the allocation of one .8 FTE Public Health Nurse effective immediately.

21.               Reclassification of Deputy Mental Health Director-Programs, and Deputy Director-Social Services; and Allocation of a Full-Time Deputy Branch Director Position in the Public Health Branch

RECOMMENDATION: Delete the class of Deputy Mental Health Director-Programs from the Classification Plan.  Adopt the management class of Deputy Branch Director into said plan.  Reallocate the Deputy Mental Health Director-Programs, currently occupied by Jet Kruse, to Deputy Branch Director.  Reallocate the Deputy Director-Social Services, currently occupied by Donna Wheeler, to Deputy Branch Director.  Allocate a full-time Deputy Branch Director in Budget Unit 400 effective the beginning of the bi-weekly pay period following approval.

Health and Human Services - Mental Health Branch

22.               Amendment to Contract with Alcohol Drug Care Services (ADCS) for the Provision of Residential Alcohol and Other Drug Treatment, Recovery, and Detoxification Services

RECOMMENDATION: Approve contract amendment, which increases the maximum funding for ADCS from $102,000 to $158,000 due to increased enrollment in treatment services through the Prop. 36 program.

23.               Appointments to the Alcohol and Drug Advisory Board

RECOMMENDATION: Appoint Miriam Hall, Guy Sovereign, Linda Jackson, Christine Burwell, Chris Wilson, and Kirk Felhaber.

Health and Human Services - Public Health Branch

24.               Approval of State Agreement No. 04-35608; Housing Opportunities for Persons with AIDS Program

RECOMMENDATION: Approve agreement for receiving $147,960 in funding.

25.               Resolution Authorizing the Director of the Environmental Health Division to Submit a Grant Application to the California Integrated Waste Management Board Each Year for a Maximum Period of Five Years

RECOMMENDATION: Adopt Resolution No. 05-__.

26.               Request for Approval of a Supplemental Budget for Fund 1175, Budget Unit 410, and Approval of the Revised Maddy Emergency Medical Services (EMS) Fund Participation Procedures

RECOMMENDATION: Approve request, and approve adoption of a uniform fee schedule set at 125% of the allowable Medicare procedure charges.

27.               Agreement for Medi-Cal Targeted Case Management Between Department of Health and Human Services, Public Health Branch, and Humboldt County Probation department for Fiscal Year 2004, 2005, and 2006

RECOMMENDATION: Approve agreement.

28.               Amendment to Grant Agreement with First 5 Humboldt for Service Enhancement to Families with Children Ages 0 to 5

RECOMMENDATION: Amendment agreement to extend same for one year and to revise the scope of work and budget.  Adopt a supplemental budget ($32,533).

32.               Evaluation of Two Proposed Names for Humboldt County Bridge No. 4C-156

RECOMMENDATION: Deny the request from Ms. September Gray to officially name Humboldt County Bridge #4C-156 "Wailaki Pass Bridge."  Deny the request from the Garberville-Redway Area Chamber of Commerce to officially name Humboldt County Bridge #4C-156 after Mr. Charles Wood.  Direct staff to send letters to Ms. Gray and the Garberville-Redway Area Chamber of Commerce informing them of the Board's decision.

33.               Amendment to Lease Agreement with Johnson's Mobile Solutions for Rental of Modular Unit for Natural Resources Division

RECOMMENDATION: Approve amendment to extend lease for an additional 36 months.

34.               Letter to the Sonoma County Water Agency Commenting on the Scope of the Notice of Preparation for the Environmental Impact Report

RECOMMENDATION: Authorize Chair to sign said letter.

35.               Request to Set Hearing Date for Unmet Transportation Needs

RECOMMENDATION: Approve supplemental budget ($2,713) to correct the appropriation in Citizen Corps expenditure account.

37.               Supplemental Budget for the Purchase of Four Computers for the Records Management Division

RECOMMENDATION: Adopt supplemental budget ($7,000).

38.               Supplemental Budget for Additional Costs to Replace Deputy's Body Armor

Community Development Services (CDS)

1.                   Shore Zone Reclassification/Zone Boundary Adjustment/Joint Timber Management Plan Application; File No. 522-142-08 et al.  Case No. ZR-03-08/JTMP-03-02; Willow Creek

RECOMMENDATION: Approve the Zone Reclassification amending Section 311-7 of the Humboldt County Code by reclassifying an approximate 40 acres of lands currently zoned Agriculture Exclusive (AE) into Timberland Production

Zone (TPZ), and adopt Ordinance No. _______.  Approve the Zone Boundary Adjustment to adjust the zone boundary between Timberland Production Zone (TPZ) and Agriculture General (AG-B-7(2.5)) to be coterminous with the common property line between APN's 522-143-01 and -23 as adjusted by LLA-02-35.  Direct CDS staff to prepare and file a Notice of Exemption pursuant to CEQA for the project.  Direct the Clerk of the Board to record a Notice of Timberland Production Zone Status on the parcel pursuant to Sections 51117 and 51141 of the California Government Code.

2.                   Charley Zone Boundary Adjustment; File No. APN 312-131-06 and -36; Case No. ZR-04-01; Blue Lake

RECOMMENDATION: Approve the Zone Boundary Adjustment amending Section 311-7 of the Humboldt County Code by adjusting the zone boundary of the Unclassified Zone (U) to be coincidental with the proposed boundaries of Parcel B, and to adjust the zone boundary of the Agriculture Exclusive/Timberland Production Zone (AE-B-7(42)/TPZ-B-7(42)) to include the 0.8 acres being adjusted into Parcel A, said Parcels A and B as described by LLA-04-01, and adopt Ordinance No. ________.  Direct CDS staff to prepare and file a Notice of Exemption pursuant to CEQA for the project.

3.                   Gross Zone Reclassification/Lot Line Adjustment; File No. APN 402-091-20; Case No. ZR-04-02; Freshwater Area

RECOMMENDATION: Approve the Zone Reclassification, amending Section 311-7 of the Humboldt County Code by reclassifying

an approximate 0.36 acre from Agricultural General (AG) into Agricultural General with a Special Building Site Combining Zone specifying a minimum 5 acres (AG-B-5(5)), and adopt Ordinance No. ____.  Direct CDS staff to prepare and file a Notice of Exemption pursuant to CEQA for the project.
