RECOMMENDATION: Establish two new trust funds as follows:  3723 DNA Identification Fund, and 3724 Local DNA Identification Fund.

2.                   Continuation of Emergency - Transmission of Disease Through Contaminated Needle Use (Supervisor Rodoni)

RECOMMENDATION: Declare that a local state of emergency, per Resolution No. 2000-14, remains in effect until June 14, 2005, allowing the Public Health Branch to continue a needle-exchange program.

3.                   Certificate of Recognition for Norman "Joe" Toroni, Fortuna Chamber of Commerce 2004 Citizen of the Year (Supervisor Rodoni)

RECOMMENDATION: Ratify the Certificate of Recognition.

4.                   Certificate of Recognition for PaLCo, Fortuna Chamber of Commerce 2004 Business of the Year (Supervisor Rodoni)

RECOMMENDATION: Ratify the Certificate of Recognition.

5.                   Resolution of Commendation for Pam Gianotti on Her Retirement from the Humboldt County Assessor's Office (Supervisor Rodoni)

RECOMMENDATION: Adopt the Resolution.

6.                   Resolution of Commendation for Don Gianotti on His Retirement from the Humboldt County Information Services Department (Supervisor Rodoni)

RECOMMENDATION: Adopt the Resolution.

7.                   Resolution Recognizing the Name and Status Change of the Humboldt Child Trauma Council to the Child Abuse Prevention Coordinating Council of Humboldt County (Supervisor Rodoni)

RECOMMENDATION: Adopt Resolution No. 05-31.

8.                   Letter of Support for Assembly Bill 7 (Cogdill) - Sports Fishing, License Revenues; Inland Fisheries (Supervisor Geist)

RECOMMENDATION: Authorize Chair to sign the letter of support.

9.                   Local Child Care Planning Council (LPC) State and Federal Funding Priorities (Supervisor Neely)

RECOMMENDATION: Authorize Chair to sign the Priorities Report Form.

10.               Assistance for the Historic Preservation Workshop for Northcoast Communities on June 24, 2005 (Supervisor Woolley)

RECOMMENDATION: Approve assistance in the amount of $125.00; direct the Auditor to establish line item 1100-101-2123, Special Department Expense; and approve the appropriation transfer of $25.00, respectively, from 1100-101-2291, 2292, 2293, 2294, and 2295 to 1100-101-2123.

RECOMMENDATION: Approve the Board Orders.

Community Development Services

13.               Resolution Accepting Program Year 2005-06 Title I WIA Subgrant Agreement; Registration No. R692474

RECOMMENDATION: Adopt Resolution No. 05-32; authorize Chair to sign subgrant; and authorize the Workforce Investment Board Executive Director to sign administrative documents (as required by the State of California Employment Development Department).

Health and Human Services - Administration

14.               Allocation of Administrative Analyst I/II Position in Budget Unit 424

RECOMMENDATION: Approve allocation.

15.               Reallocation of Medical Office Assistant I/II Position to an Administrative Secretary Position in Budget Unit 424

RECOMMENDATION: Approve reallocation.

16.               Agreement with Humboldt Child Care Council for Family Preservation Services

RECOMMENDATION: Extend existing agreement through June 30, 2006.

Health and Human Services - Mental Health Branch

17.               Transfer of $5,500 for Purchase of Rubber Mats for the Outside Play Area at the Healthy Moms Program

RECOMMENDATION: Approve transfer.

18.               First Amendment to Combined Negotiated Net Amount and Drug Medi-Cal Contract for Fiscal Year 2004-05

RECOMMENDATION: Approve first amendment to said contract, which increases the revenue of Humboldt County's Mental Health Alcohol and Other Drug Programs by $62,955 from $1,344,706 to $1,407,661 for Fiscal Year 2004-05.

Health and Human Services - Public Health Branch

19.               Agreement for Childhood Lead Poisoning Prevention Program

RECOMMENDATION: Approve agreement with the California Department of Health Services.

Health and Human Services - Social Services Branch

20.               Agreement with the Humboldt Child Care Council

RECOMMENDATION: Approve agreement to provide stage one child care services to all CalWORKs recipients that are referred by the Social Services Branch.

Library Services

21.               Donations from the Friends of the Arcata Library, Mad River Farm, and Individuals

RECOMMENDATION: Accept donations, totally $1,295, for the purchase of new books and to cover recruitment costs for a retiring librarian and travel expenses related to conference attendance for the Circulation Manager.


22.               Application to the California Board of Corrections for Funding Under the Juvenile Accountability Block Grant Program

RECOMMENDATION: Authorize the Chief Probation Officer to sign necessary documents for submission of the application for funding as well as grant award agreements, necessary contracts, agreements, and budget detail in order to accept the grant funding and implement the projects and activities described in the application for funding, subject to the review and approval of the CAO, Risk Manager and County Counsel.

Public Works

23.               Resolution Authorizing the Department of Public Works to Enter Into an Agreement on Project Procedures with the Federal Highway Administration for Roads Within the Forest Highways System

RECOMMENDATION: Adopt Resolution No. 05-__.

24.               Extension of Lease for Public Works Sign Shop and Vehicle Storage

RECOMMENDATION: Approve a one-year extension of the lease with Charlene and Ron Lundblade for property located at 112 W. Third Street in Eureka.

25.               Storm Damage Repairs on Bald Mountain Road; Project No. CDAA-2003-01; Contract No. 203155

RECOMMENDATION: Approve plans and specifications, and open bids on June 21, 2005, at 2:00 p.m.

26.               Security Fence Enhancement at Murray Field Airport; Project No. AIP 3-06-0072-06; Contract No. 914151

RECOMMENDATION: Award the project to Miller Farms Nursery.

27.               Kneeland Airport Runway Rehabilitation Project; Project No. AIP 3-06-0362-04; Contract No. 914141

RECOMMENDATION: Award the project to Mercer Fraser Company.

28.               Dinsmore Airport Drainage and Safety Area Enhancements; Project No. AIP 3-06-0361-04; Contract No. 914121

RECOMMENDATION: Award the project to Mercer Fraser Company.

29.               Larsen Subdivision Final Map; Assessor's Parcel No. 511-443-01 (Formerly 511-447-87); McKinleyville Area

RECOMMENDATION: Declare the Larsen Subdivision as complete, and accept and approve the final map for filing.  Accept the offers of dedication of Parcels A 1, A2, and L for public road purposes; Parcels F, J, M, and N for public utilities; Parcels G and H for pedestrian purposes; Parcels K and R for drainage; and reject Parcel A3 for

public road purposes, all as offered to the County of Humboldt as shown on the subdivision map.  Authorize the Chair to execute the Conveyance and Agreement.

Authorize the Chair to accept the Overflight Easement granted to the County of Humboldt.  Direct the Clerk of the Board to sign the subdivision map on behalf of the Board and transmit the subdivision map. Conveyance and Agreement, and Overflight Easement Deed to the Recorder for recordation.


30.               Supplemental Budget to Purchase a Commercial Washing Machine for the Correctional Facility

RECOMMENDATION: Adopt supplemental budget ($8,409).

1.                   Ordinance Amending Sections 547-11 and 547-13 of the Humboldt County Code Relating to the Appeal of Findings of Potentially Dangerous and Vicious Dogs

RECOMMENDATION: Introduce ordinance by title, waive the first reading, and set the ordinance for adoption on June 7, 2005.

County Administrative Office (CAO)

RECOMMENDATION: Receive update, and take appropriate action (if required).

2.                   Standing Item--Pursuant to Government Code Section 54957.6, Review the County's Position, and Instruct Designated Representatives with Respect to the Various Forms of Compensation Provided to the County's Represented and Unrepresented Employees   (if required)

3.                   Standing Item--Report on Pending or Threatened Litigation Pursuant to Government Code Section 54956.9   (if required)

4.                   It is the intention of the Board of Supervisors to meet in closed session to consider the public employee performance evaluation for the position of Personnel Director pursuant to Government Code Section 54954.5.

5.                   Standing Item--Adjourn as County Board of Supervisors, and Reconvene as Humboldt County Public Authority.  Pursuant to Government Code Section 54957.6, Conference with Labor Negotiator.  It is the intention of the Board of the Humboldt County Public Authority to meet in Closed Session to have a conference with its Labor Negotiator to review and instruct designated representatives with respect to the various forms of compensation provided to represented and unrepresented personnel of the Humboldt County Public Authority.

RECOMMENDATION: Confirm the Findings and Order confirming account and assessment of $11,838.69.  Authorize the Chair of the Board to sign the Findings and Order confirming account and assessment.
