3.                   Set Aside Previous Approvals in the Klamath River Aggregates Surface Mining Project in Conformance with Settlement Agreement

RECOMMENDATION: Review the Stipulation for Judgment and Judgment filed February 7, 2005 in Case No. 040467 (submitted Attachment A) recalling that the parties (including the County of Humboldt) agreed to settle the pending writ based upon the convenience of the parties.  In conformance with the settlement agreement, set aside the decision made on May 25, 2004, wherein the Board adopted a mitigated negative declaration and issued permit approval for the project described as the Klamath River Aggregates Surface Mining Conditional Use Permit [Case No. CUP-02-15/SMP-02-01/RP-02-01] (Ammon Quarry), based upon

Klamath River Aggregates willingness to participate in the preparation of an environmental impact report.  Direct County Counsel's office to file a Return with the Humboldt County Superior Court showing that the decision has been set aside.

4.                   Proposed Ordinance Amending Sections 245-1, 245-2, and the Title of Chapter 5 of Title II of the Humboldt County Code, Relating to the Reorganization of the General Services Department and County Administrative Office

RECOMMENDATION: Adopt Ordinance No. ____.

General Services

5.                   Reimbursement from the California State Association of Counties-Excess Insurance Authority for the Purchase of Scanning Software and Equipment

RECOMMENDATION: Accept reimbursement ($8,130).

Health and Human Services - Administration

6.                   Increase of FTE of A Community Health Outreach Worker Position in Budget Unit 414

RECOMMENDATION: Increase the FTE of Community Health Outreach Worker Position 02, currently occupied by Megan Evans, from .6 FTE to 1.0 FTE effective the first bi-weekly pay period following approval.

7.                   Reduction in Force:  Disallocation of One Health Education Specialist Position in Budget Unit 414.

RECOMMENDATION: Authorize disallocation of Health Education Specialist II, class #1595, position #07, in budget unit 414, effective July 15, 2005; and authorize the Personnel Department to begin the layoff process for the above disallocation.

8.                   Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve extension for Laurie Sims, Clinician I, to a maximum of 1,250 hours.  Approve extension for Omar Daniel, Mental Health Worker I, to a maximum of 1,440 hours.

9.                   Resolution Approving Humboldt County's Fiscal Year 2005-2006 Plan for the Substance Abuse and Crime Prevention Act of 2000 Program

RECOMMENDATION: Adopt Resolution No. 05-__, indicating approval of the plan, agreeing to comply with all provisions of the Act, and assuring maintenance of a trust fund for funds received pursuant to the Act.

10.               Second Amendment to the Service Agreements with the Humboldt Child Care Council for the Provision of Mental Health and Case Management Services and Therapeutic Behavioral Services to Medi-Cal Beneficiaries to Adjust the Contract Value for Fiscal Year 2005-06

RECOMMENDATION: Approve amendments to increase case managements services to $850,000 per fiscal year and reducing therapeutic behavioral services to $150,000 per fiscal year.

11.               Second Amendment to the Service Agreement with the Humboldt Family Service Center for the Provision of Specialty Mental Health Services to Eligible Humboldt County Medi-Cal Clients to Adjust the Contract Value for Fiscal Year 2005-06

RECOMMENDATION: Approve second amendment, decreasing the contract value for Fiscal Year 2005-06 from $135,000 per fiscal year to $114,200 per fiscal year.

Health and Human Services - Public Health Branch

12.               Amendment to Agreement with First 5 Humboldt for the Best Beginnings for Moms and Babies Project

RECOMMENDATION: Approve amendment, which extends agreement to September 30, 2005.

13.               Third Amendment to Lease with Al Abrahamsen, Alice F. Abrahamsen, Kent H. Pryor and Alice Pryor for Office Space Located at 317 Second Street in Eureka

RECOMMENDATION: Approve amendment, extending the lease through June 30, 2008, at an additional cost of $100 per month.

14.               Resolution Supporting Humboldt County's Application to the California Integrated Waste Management Board to Receive Fiscal Year 2005-06 and the Subsequent Five Years of State Funding for Costs Related to Operation of Humboldt County's Regional Used Oil Recycling Program

RECOMMENDATION: Adopt Resolution No. 05-__.

15.               Amendment to California Forensic Medical Group Contract for Provision of Jail Medical Services

RECOMMENDATION: Approve third contract amendment that extends the contract from July 1, 2005, through June 30, 2007, and provides an adjustment to the annual base price.

Health and Human Services - Social Services Branch

16.               Purchase of Two Off-Road Vehicles, One Automobile, One 7-Passenger Van, and Computer Items (Lap Tops, Equipment and Furniture) for the CalWorks Program ($691,000)

RECOMMENDATION: Approve purchase.


17.               Reimbursement of $880 from the Department of Health and Human Services

RECOMMENDATION: Accept reimbursement to cover the cost of sales tax and environmental fees for three computers and supplies needed for recruitment bulletins.

Public Works

18.               Resolution Authorizing the Annual Adjustment of the Funds Pledged by the Public Works Department to Complete Reclamation of Rock and Gravel Quarry Sites

RECOMMENDATION: Adopt Resolution No. 05-__.

19.               First Amendment for Extension of the Agreement with the Humboldt Waste Management Authority to Provide Staff Services for Administration of the County Solid Waste Franchise Contracts and Rural Container Site System

RECOMMENDATION: Approve amendment to extend agreement through June 30, 2007.

20.               Construction of Saunders Creek Culvert Replacement on Mattole Road; Contract No. 203303

RECOMMENDATION: Award the project to Granite Construction Company.

21.               Construction of Storm Damage Repairs on Central Avenue; Contract No. 203156

RECOMMENDATION: Award the project to Grant Equipment.

22.               Construction of Storm Damage Repairs on Benbow Drive; Contract No. 203142

RECOMMENDATION: Award the project to G. R. Wilcox Enterprises, Inc.

23.               Proposed Ordinance Amending Section 431-65(9) of the Humboldt County Code, Relating to Parking on School Road (Requesting the Elimination the Parking of Vehicles on School Road Along the Frontage of the New Veterinary Facility and Adjacent Property Across the Road from K-Mart)

RECOMMENDATION: Adopt Ordinance No. ____.

24.               Construction of Briceland Thorne Road (F5A010) Between PM 5.50 and 5.65, Project No. STPLH-5904 (058), Contract No. 203851.

RECOMMENDATION: Approve the plans and specifications.  Direct the Clerk to advertise the above-mentioned project as required by Section 22037 of the California Public Contract Code with Bids to be opened on Tuesday, June 28, 2005 at 2:00 p.m.


25.               Proposed Ordinance Amending Sections 547-11 and 547-13 of the Humboldt County Code Relating to the Appeal of Findings of Potentially Dangerous and Vicious Dogs

26.               Classification Review--Assistant Treasurer/Tax Collector and Senior Treasury and Tax Assistant

RECOMMENDATION: 1.  Adopt the Management Class of Assistant Treasurer/Tax Collector (salary range 395, class 0682, unit 8) into the classification plan.

2.  Approve the reallocation of the Supervising Tax Collector position currently occupied Velma Thiel in budget unit 112, (salary range 333, class 0674) to the class of Assistant Treasurer/Tax Collector (salary range 395, class 0682).

3.  Delete the class of Supervising Tax Collector, (salary range 333, class 0674).

4.  Adopt the forty-hour class of Senior Treasury and Tax Assistant (salary range 303, class 0679, unit 1) into the classification plan.

5.  Approve the reallocation of the Treasury and Tax Assistant I/II position currently occupied by Lenora Viggers in budget unit 112, (salary range 226/246 class 0170, position number 2) to the class of Senior Treasury and Tax Assistant (salary range 303, class 0679).

6.  Approve advanced step placement to step C for Senior Treasury and Tax Assistant, Lenora Viggers effective June 19, 2005 pursuant to Section 7 of the Humboldt County Salaries Resolution.
