RECOMMENDATION: Approve agreement for $210,806 in grant funding for property tax administration enhancement.

2.                   Resolution Congratulating the California Conservation Corps on its 25th Anniversary (Supervisor Smith)

RECOMMENDATION: Ratify the Resolution.

Community Development Services--Economic Development Division

3.                   Request for Review and Approval of a Waiver of Nuclear-Weapons-Free-Zone Clause with the Regents of the University of California (On Behalf of its Cooperative Extension Humboldt County) in a Contract for a Natural Meat Feasibility Report

RECOMMENDATION: Approve request.

District Attorney

4.                   Extension of Lease at 712 Fourth Street, Eureka--Victim Witness Office Space

RECOMMENDATION: Approve extension of lease through June 30, 2006.

Health and Human Services - Mental Health Branch

5.                   Standard Agreement with the California Department of Rehabilitation for the Provision of Employment Services for Persons with Severe Psychiatric Disabilities

RECOMMENDATION: Approve agreement.

6.                   Agreement with Alcohol Drug Care Services (Bonnie Brown Program)

RECOMMENDATION: Approve agreement for the provision of non-medical residential alcohol and other drug treatment and recovery services for women.

7.                   Second Amendment to the Service Agreement with Catholic Charities of the Diocese of Santa Rosa for the Provision of Specialty Mental Health Services to Eligible Humboldt County Medi-Cal Clients to Adjust the Contract Values for Fiscal Years 2004 and 2005

RECOMMENDATION: Approve second amendment.

Health and Human Services - Public Health Branch

8.                   Agreement with the California Department of Health Services

RECOMMENDATION: Approve agreement for allocation of $30,000 to support efforts to facilitate, develop, and enhance local capacity for Chlamydia testing in the Humboldt  County Juvenile Hall.

9.                   Agreement with the California Department of Health Services

RECOMMENDATION: Approve agreement for $109,482 in funding to raise immunization levels among kindergarten entrants and incoming transfer students, infants, and preschool-age children; and to eliminate illness, disability, and death due to vaccine-preventable diseases through prevention, surveillance, and outbreak control.

10.               Request for Proposals (RFP) for Leased Space for the Health Education/Prevention Division

RECOMMENDATION: Approve RFP, and open proposals on Friday, August 12, 2005, at 2:00 p.m.

Health and Human Services - Social Services Branch

11.               Memorandum of Understanding (MOU) with Umpqua Bank to Provide Low-Interest Car Loans to CalWORKs/WtW Participants

RECOMMENDATION: Authorize Chair to execute MOU.

12.               2005-08 Cost-Reimbursement Contract with the Humboldt Child Care Council

RECOMMENDATION: Approve contract for providing child-care services to eligible WIA clients.

Personnel

13.               Renewal of Test-Use Agreement with Cooperative Personnel Services

RECOMMENDATION: Authorize Personnel Director to execute said agreement.

Public Works

14.               Services Agreements for Fieldbrook and Redway Monument Preservation Fund Surveys

RECOMMENDATION: Award the Fieldbrook Monument Preservation Fund Project to Hunt Surveying & Forestry, Inc.  Award the Redway Monument Preservation Fund Project to Barry L. Kolstad, Professional Land Surveyor.  Approve agreements, and adopt supplemental budget ($92,000).

15.               Construction of Storm Damage Repairs on Wilder Ridge Road; Contract No. 203190

RECOMMENDATION: Approve the plans and specifications, and hold bid opening on Tuesday, July 19, 2005, at 2:00 p.m.

16.               Structural Modifications to Eel River Bridge on Fort Seward Road; Contract No. 594004

RECOMMENDATION: Approve the plans and specifications, and hold bid opening on Tuesday, July 19, 2005, at 2:00 p.m.

17.               Construction of Pedestrian/Bicycle Improvements on Walnut Drive; Contract No. 200701

RECOMMENDATION: Approve the plans and specifications, and hold bid opening on Tuesday, July 19, 2005, at 2:00 p.m.

18.               Annual Disadvantaged Business Enterprise (DBE) Goal for Federally-Funded Transportation Projects

RECOMMENDATION: Receive and file the DBE program, and establish a proposed goal of 5.7% for Federal Fiscal Year 2005-06.

In-Home Supportive Services Public Authority

1.                   Agreement with the Area I Agency on Aging for an In-Home Supportive Services Provider Registry

RECOMMENDATION: Adjourn as Humboldt County Board of Supervisors.  Convene as the Governing Board of the Humboldt County In-Home Supportive Services Public Authority.  Approve said agreement.  Reconvene as Humboldt County Board of Supervisors.

1.                   Resolution Authorizing Submission of a $70,000 Grant Application for the 2005-06 CDBG from the Planning and Technical Assistance Round for an Implementation Plan for Deploying Wireless Broadband to the Willow Creek Area and a Water Supply Upgrade Feasibility Study for the Community of Loleta

RECOMMENDATION: Adopt Resolution No. 05-__.
