RECOMMENDATION: That the Board of
Supervisors approve the Board Orders.

Supervisors approve the Board Orders.
RECOMMENDATION: That the Board of
Supervisors adopt the attached resolution supporting the findings of the
California Statewide Local Streets and Roads Needs Assessment.

1.
Certificates
of Recognition for CHP Officer Dave Anderson and Cody Box.

RECOMMENDATION: Adopt the Certificates of
Recognition.

CONSENT CALENDAR
These matters are routine in nature and are usually
approved by a single vote. Prior to the meeting, any member of the public may
request any member of the Board to pull a particular item for further
discussion.
Agricultural Commissioner
RECOMMENDATION: That the Board of
Supervisors approve Standard Agreement No. 09-0496, Known as the High-Risk Pest
Exclusion Program Contract, and authorize the Chair to sign the original and
one copy of the cover page as well as the Contractor Certification form, and
return all copies to the Agriculture Department for Further processing; grant a
waiver from the provisions of the Nuclear Free Ordinance, and authorize the Auditor-Controller
to execute the attached supplemental budget for fiscal year 2009-10.
RECOMMENDATION: That the Board of
Supervisors approve Standard Agreement No. 09-0595, known as the Light Brown
Apple Moth trapping contract; authorize the Chair to sign the original
agreement and one copy of the cover page as well as the Contractor
Certification form and return all copies to the Agriculture Department for
further processing; grant a waiver from the provisions of the Nuclear Free
Ordinance; and authorize the Auditor-Controller to execute the attached
supplemental budget for 2009-2010.

Nancy Koke to the Orick Community Services District Board of Directors. .

RECOMMENDATION: Appoint Nancy Koke to the
Orick Community Services District Board of Directors for a 4-year term.

Letter
of Appreciation to Robin Wolff for Serving on the Humboldt County Mental Health
Board. (Supervisor Clendenen)
RECOMMENDATION: Authorize Chair to Sign
Letter of Appreciation.
5.
Appoint
Kenna Adame to the Humboldt County Human Rights Commission. (Supervisor
Neely)
RECOMMENDATION: Approve the appointment.
6.
Resolution
to Support Regional Broadband Efforts.
(Supervisor Lovelace)
RECOMMENDATION: Adopt the Resolution.
Community Development Services
RECOMMENDATION: That the Board of
Supervisors approve proposed Title III County Projects and direct Community
Development Services - Planning Division to report back to the Board on an
annual basis with project implementation progress.
County Administrative Office
RECOMMENDATION: Approve designation of the
County Risk Manager as primary representative to the CSAC-EIA Board of
Directors and the County Administrative Officer, as alternate.
9.
Set
Public Hearing for Adoption of Fee Updates.
RECOMMENDATION: That the Board of
Supervisors direct the Clerk of the Board to schedule a public hearing on
Tuesday, January 26, 2010, at 1:30 p.m. or as soon thereafter as the matter may
be heard, for consideration of adoption of an ordinance updating the Humboldt
County Schedule of Fees and Charges; direct the Clerk of the Board to post and
publish the required pre-adoption summary of the ordinance and public hearing
notice for updating the Humboldt County Schedule of Fees and Charges at least
10 days prior to the public hearing; two publications are required, with at least
five days intervening between the dates of the first and last publication, not
counting such publication dates; and direct the Clerk of the Board to have the
data and documentation supporting the new fees for public review at least ten
days prior to the public hearing; and direct the Clerk of the Board to post a
certified copy of the full text of the proposed ordinance in the office of the
Clerk of the Board meeting at which the ordinance will be adopted [Government
Code Section 25124(b)(1)].
Health and Human Services - Administration
10.
Extension
of Extra Help Hours Past the 960 Hour Limit.
RECOMMENDATION: That the Board of
Supervisors approve the extension of extra help hours to 1984 hours through June
30, 2010, per Section 7 of the Humboldt County Salary Resolution for the
following people :
Kenneth Barney, Office Assistant I; Colby
Beach, Mental Health Worker I; Janelle Schaller, Mental Health Worker I; James
Close, Office Assistant I; Rebecca Laloli, Fiscal Assistant I; George Parlet,
Laborerel and Darin Talcott, Substance Abuse Counselor I.
Health and Human Services - Mental Health
Branch
11.
Reappointments
to the Humboldt County Mental Health Board.
RECOMMENDATION: That the Board of
Supervisors reappoint Tim Ash, Eric Thompson and John Hill to three year terms
on the Humboldt County Mental Health Board.
Health and Human Services - Public Health
Branch
RECOMMENDATION: That the Board of
Supervisors approve and Accept the Grant Award from Coast Central Credit Union;
and direct the Auditor-Controller to supplement the Budget in Fund 1175, budget
Unit #414 Health Education (HE) for Fiscal Year 2009-10 as follows; Revenue -
707-254 Other Grants $13,000: Expenditure 2-456 Safe Communities Program
$13,000.
RECOMMENDATION: That the Board of
Supervisors approve and authorize the Chair to sign three originals of the
Service Agreement with Health Monitoring Services, Inc. for the use of the
EpiCenter Service, Electronic Health Monitoring System; Direct the Clerk of the
Board to return three (3) signed originals to the Public Health Branch for
submission to Health Monitoring Services, Inc. for execution.
RECOMMENDATION: That the Board of
Supervisors approve and execute Standard Agreement Amendment 09-11272-A01 with
the California Department of Public Health to incorporate a new Supplemental
Terms and Conditions for Contracts Using American Recovery and Reinvestment Act
(ARRA) Funds; and direct the Clerk of the Board to return (7) seven signed
copies to Public Health for submission to the State.
RECOMMENDATION: That the Board of
Supervisors approve the revised class of Health Program Manager to that of HHS
Program Services Coordinator; Adopt the Non - Management class of HHS Program
Services Coordinator; Approve the Upward Reallocation of One Vacant Senior
Health Program Coordinator Position; and Delete the Class of Senior Health
Program Coordinator in Budget Unit #460, Public Health effective the beginning
of the first biweekly pay period following approval.
RECOMMENDATION: That the Board of
Supervisors approve Amendment Number A02 to Agreement No 07-65525 with the
California Department of Public Health; direct the Chair of the Board to sign
the first page of the seven standard agreement amendment packages; and direct
the Clerk of the Board to return the seven standard agreement amendment
packages to the Public Health Branch.
Health and Human Services - Social Services
Branch
RECOMMENDATION: That the Board of Supervisors
adopt the 40 hour class of Senior Welfare Investigator (salary range 423, class
0741, unit 6) into the classification plan; and allocate one full-time position
of Senior Welfare Investigator (salary range 423, class 0741) in budget unit
511 effective immediately.

RECOMMENDATION: That the Board of
Supervisors adopt the attached Humboldt county Coroner's Office Policy and
Procedures and authorize the Coroner to supplement or amend the document as
needed.

2.
Designation
of a No-Parking Zone on Letz Road (County Road 4M170), McKinleyville Area.
RECOMMENDATION: That the Board of
Supervisors introduce the attached Ordinance by Title, waive the first reading
thereof, and set the Ordinance for adoption at least one week away, and direct
the Clerk of the Board to publish a summary at least five days before the date
of adoption, and again within 15 days after adoption.

RECOMMENDATION: That the Board of
Supervisors open the public hearing and receive the staff report and public
comment; accept the petition by approving the attached resolution based on the
findings in the staff report and testimony received about the project; and
direct the Clerk of the Board to give notice of the decision to the applicant,
the Assessor's office and any other interested party.
