Board of Supervisors

1.                   Certificates of Achievement for Elliott Langer and Corey Griffin for Achieving the Rank of Eagle Scout. (Supervisor Smith)

RECOMMENDATION: Ratify Certificates of Achievement.

2.                   Tina Nerat Resolution of Recognition for her work on Broadband Communications in Humboldt County. (Supervisor Smith)

RECOMMENDATION: Ratify the Resolution.


3.                   Letters of Appreciation to Bill Davidson and Dax Williamson for their service on the Humboldt County Airport Advisory Committee. (Supervisor Clendenen)

RECOMMENDATION: Accept the resignations of Bill Davidson and Dax Williamson from the Humboldt County Airport Advisory Committee, and authorize Chair to sign letters of appreciation thanking them for their service.

4.                   Resolution Recognizing the 350th Anniversary of St. Vincent de Paul and the Local Humboldt County St. Vincent de Paul Stores. (Supervisor Neely)

RECOMMENDATION: Ratify the Resolution.

Clerk of the Board

5.                   Approval of the Board Orders - Meeting of October 5, 2010.

RECOMMENDATION: That the Board of Supervisors approves the Board Orders and authorizes the Clerk of the Board and the Board Chair to sign the minute book in accordance with California Government Code Section 25103.

6.                   Approval of the Board Orders Meeting September 21, 2010.

RECOMMENDATION: That the Board of Supervisors approve the Board Orders of the September 21, 2010.

7.                   Approval of Board Orders of September 14, 2010.

RECOMMENDATION: That the Board of Supervisors approve the Board Orders of September 14, 2010.

8.                   Approval of Board Orders for the Meeting of June 22, 2010

RECOMMENDATION: That the Board of Supervisors approve the Board Orders from the Meeting of June 22, 2010.

9.                   Approval of the Board Orders - Meeting of May 18, 2010.

RECOMMENDATION: That the Board of Supervisors approve the Board Orders from the Meeting of May 18, 2010.

County Administrative Office

10.               Letter of Support for Humboldt Bay Municipal Water District (HBMWD) Blue Lake/ Fieldbrook Pipeline Replacement Project.

RECOMMENDATION: That the Board of Supervisors authorizes the Chair to sign the attached letter supporting HBMWD in its efforts to secure Pre Disaster Mitigation Grant Program funding for the Blue Lake/Fieldbrook Pipeline Replacement Project.

Health and Human Services - Administration

11.               Ratification of Termination for Convenience of the General Relief Shelter Program Agreement with Alcohol Drug Care Services (ADCS). (Consent)

RECOMMENDATION: That the Board of Supervisors ratifies termination of the General Relief Shelter Program Agreement with ADCS for the provision of the General Relief Shelter Program.

Health and Human Services - Public Health Branch

12.               Approve and Authorize the Memorandum of Understanding (MOU) between the Public Health Branch and Catherine L. Zane Middle School for a Student Health and Nutrition Project during the 2010-11 School Year.

RECOMMENDATION: That the Board of Supervisors approves and authorizes the Chair to sign three (3) original MOU between the Department of Health and Human Services, Public Health Branch and Catherine L. Zane Middle School for the period of October 1, 2010 through September 30, 2011. Direct the Clerk of the Board to return three (3) signed originals of the MOU to the Department of Health and Human Services, Public Health Branch.

13.               Adoption of Ordinance Amending Sections 551-2, 551-4, 551-5, 551-8, 551-9, 551-10 and 551-11 of Title V of the Humboldt County Code relating to Emergency Services System and Regulation of Ambulance Service.

RECOMMENDATION: That the Board of Supervisors adopts Ordinance No. ____ amending Sections 551-2, 551-4, 551-5, 551-8, 551-9, 551-10, and 551-11 of Title V of the Humboldt County Code relating to Emergency Services System and Regulation of Ambulance Service, pursuant to the attached Board Order.

Public Works

14.               Transportation Development Act Claim.

RECOMMENDATION: That the Board of Supervisors authorizes the Chairperson to sign the attached Transportation Development Act (TDA) Claim in the amount of $1,836,889 and direct the Clerk of the Board to forward the claim to the Humboldt County Association of Governments (HCAOG).

15.               State Transit Assistance Fund Claim.

RECOMMENDATION: That the Board of Supervisors authorizes the Chairperson to sign the attached State Transit Assistance Fund (STAF) Claim in the amount of $60,098 and direct the Clerk of the Board to forward the claim to the Public Works Department for further processing.

16.               Construction of Storm Damage Repairs on Trinidad Scenic Drive (4M310) at PM 0.75 & 1.00; Project No. PW-1147, Contract No. 206286 & 206364.

RECOMMENDATION: That the Board of Supervisors authorizes the Director of Public Works to sign Contract Change Order Number 1 for the total amount of $6,703.55; accept the completed contract, including all change orders approved by the Public Works Department, between GR Sundberg, Inc., Contractor, and Humboldt County for Construction of storm damage repairs on Trinidad Scenic Drive (4M310) at PM 0.75 & 1.00; and approve the attached Appropriation Transfer to provide funding.

17.               Construction of Mattole Road (F3C010) Overlay Between PM 14.0 and 14.75: Project No. RPL-5904 (073), Contract No. 202704.

RECOMMENDATION: That the Board of Supervisors authorizes the Director of Public Works to sign Contract Change Order Number 1 for the total amount of $33,197.28; accept the completed contract, including all change orders approved by the Public Works Department, between Granite Construction Company, Contractor, and Humboldt County for Construction of Mattole Road (F3C010) Overlay between PM 14.0 and 14.75; and approve the attached Appropriation Transfer to provide funding.

Sheriff

18.               Approval of 2011 Annual Operating Plan for Corporative Patrol Operations Between Humboldt County Sheriff's Office and the Six Rivers National Forest.

RECOMMENDATION: That the Board of Supervisors authorizes the Sheriff to sign two (2) originals of the Federal fiscal year (FFY) 2011 Operating and Financial Plan for Cooperative Patrol, United States Forest Service (USFS), Six Rivers National Forest - Sheriff, Humboldt County (Exhibit A); and directs the Clerk of the Board to return all documents to the Sheriff's Office for further processing. A fully executed original of each plan will be returned to the Board for filing.


19.               Approve Contract with State Department of Boating and Waterways in the Amount of $34,500 to Purchase Equipment and Supplemental Budget in Budget Unit 1100229.

RECOMMENDATION: That the Board of Supervisors approves acceptance of the Boating Safety and Enforcement Grant Equipment and Operation Contract for the purchase of equipment in the amount of $34,500; and authorizes the Chair to sign certifications incorporated in the Boating Safety and Enforcement Grant Equipment and Operation Contract; and authorizes the Sheriff to sign agreement and all necessary documents to claim reimbursements from the California Department of Boating and Waterways; and directs the Clerk of the Board to certify the approved Board Order and return all documents for processing to the Sheriff's Office, Attention Linda Modell; and approve the attached supplemental budget in Budget Unit 1100229 in the amount of $34,500.

20.               Waiver of Conditions of Section 29 B.1 and 29 C.5 (Catastrophic Leave) of the Current Memorandum of Understanding between the County of Humboldt and the Humboldt Deputy Sheriff's Organization, Law Enforcement, Non-Management, Unit 6.

RECOMMENDATION: That the Board of Supervisors approves the waiver of Section 29 B.1 (Catastrophic Leave) Conditions Under Which Paid Leave Time May Be Donated to an Employee, and allow any full-time, permanent employee, regardless of department, to donate eligible time to Deputy Sheriff Joseph Marsh as outlined in the current Memorandum of Understanding (MOU) Between the County of Humboldt and the Humboldt Deputy Sheriffs Organization, Law Enforcement, Non-Management Unit 6, Attachment A; and approves the waiver of Section 29 C.5 (Catastrophic Leave) Conditions Under Which Paid Leave Credits May Be Used, and allow the use of donated paid leave not to exceed one year for Deputy Sheriff Joseph Marsh as outlined in the current MOU Between the County of Humboldt and the Humboldt Deputy Sheriff's Organization, Law Enforcement, Non-Management Unit 6, Attachment B.

21.               Approval of Agreement for Services Between County of Humboldt and H. David Trobitz, DVM.

RECOMMENDATION: That the Board of Supervisors authorizes the Chair to sign the Agreement for Services by and between the County of Humboldt and H. David Trobitz, DVM and Heather Trobitz.

1.                   Resolution of Commendation for the District Attorney Domestic Violence Vertical Prosecution Team. (Supervisor Neely)

RECOMMENDATION: Adopt the Resolution.

Community Development Services

1.                   BAKER General Plan Petition, File No.: APNs: 202-151-13 et seq., Case No.: GPP-09-06, Fortuna Area.

RECOMMENDATION: That the Board of Supervisors opens the public hearing and receives the staff report and public comment; accepts the petition by approving the resolution (Attachment 1) based on the findings in the staff report and testimony received about the project. Direct the Clerk of the Board to give notice of the decision to the applicant, the Assessor's Office and any other interested party.
