RECOMMENDATION: That the Board of Supervisors authorize the Chairman to sign the attached Grant Application Cover Sheet.

2.                   Agreement between the Victim Compensation and Government Claims Board (VCGCB) and the County of Humboldt. (CONSENT)

RECOMMENDATION: That the Board of Supervisors adopt the resolution and provide for five (5) signed originals which are due June 28, 2011.

3.                   Request to Approve Type 20 Off-Sale Beer and Wine License for the McKinleyville Central Market. (Supervisor Sundberg)

RECOMMENDATION: Approve the application for a Type 20 Off-Sale Beer and Wine License for the McKinleyville Central Market.

4.                   Informational Item Reappointing Maggie Herbelin to the Humboldt County Human Rights Commission. (Supervisor Bass)

RECOMMENDATION: Receive and file the report on the reappointment of Maggie Herbelin to the Human Right Commission.

5.                   Informational Item Reappointing Byrd Lochtie to the Humboldt County Human Rights Commission. (Supervisor Smith)

Child Support Services

7.                   Approval of the Board Orders - Meeting of June 14, 2011.

RECOMMENDATION: That the Board of Supervisors approves the Board Orders from the meeting of June 14, 2011.

Community Development Services

8.                   Agreement between Humboldt County Community Development Services (CDS) Economic Development and Humboldt County Department of Health and human Services (DHHS) for the use of Workforce Investment Act (WIA) funds to serve Disabled Clients and provide Rapid Responses services.

RECOMMENDATION: That the Board of Supervisors receive the staff report; approve and authorize the Chairperson of the Board to sign the proposed Agreement for the provision of Workforce Investment Act funds to serve adults between the Director of the Humboldt County Community Development Services Department (CDS) and the Director of the Department of Health and Human Services (DHHS); approve and authorize the Executive Director of the Workforce Investment Board (WIB) and the Department of health and human Services, Social Services Branch Director to sign Disabled Persons Navigator (DPN) Funding Grant Implementation Plans (DPN-GIP) and Rapid Response Funding Grant Implementation Plans (RR-GIP) for all applicable funding as it becomes available; Support and affirm future collaboration between the Directors of CDS and DHHS.

District Attorney

9.                   Revolving Fund Agreement with the Victim Compensation and Government Claims Board.

RECOMMENDATION: That the Board of Supervisors direct the Chair of the Board to sign four originals of the attached agreement between the County of Humboldt and the Victim Compensation and Government Claims Board; adopt the attached resolution associated with the VCGCB agreement and direct the Clerk of the Board to process and return the executed copies to the District Attorney's Office for further processing.

Health and Human Services - Mental Health Branch

10.               Amendment Number One to the County Medical Services Program (CMSP) Participating Facility Agreement with Anthem Blue Cross.

RECOMMENDATION: That the Board of Supervisors approve Amendment Number One to the CMSP Participating Facility Agreement with Anthem Blue Cross; approve and authorize the Mental Health Branch Director to sign Amendment Number One and any subsequent amendments to this CMSP Participating Facility Agreement with Anthem Blue Cross; direct the Clerk of the Board to send one fully executed agenda item to the Department of Health and Human Services - Mental Health Branch Administration.

Health and Human Services - Public Health Branch

11.               State Standard Agreement #11-10096 between California Department of Public Health and Humboldt County for Fiscal Years 2011-2014.

RECOMMENDATION: That the Board of Supervisors approve State Standard Agreement #11-10096 with the California Department of Public Health (CDHP) for the continuation of the Childhood Lead Poisoning Prevention Program (CLPPP) in the amount of $195,692 for the period beginning July 1, 2011 and ending June 30, 2014 or upon approval by CDHP or Department of General Services (DGS) if required, whichever is later; direct the Chair of the Board to sign seven (7) originals of the cover page of the State Standard Agreement package, seven (7) originals of the Certification Regarding Lobbying located on Page 23 of Exhibit D- Special Terms and Conditions of the Agreement, and on original of the CCC307 Contractor Certification Clauses form; and direct the Clerk of the Board to return the seven originals of the State Standard Agreement package and one original of the CCC 307 Contractor Certification Clauses form to the Public Health Branch.

12.               Amended Agreement with First 5 Humboldt for the Hoopa School Readiness Program and Supplement the Budget in Fund 1175, Budget Unit #420, Maternal Child and Adolescent Health (MCAH) Coordination Project, for Fiscal Year 2010-11.

RECOMMENDATION: That the Board of Supervisors approve the amendment to original agreement with First 5 Humboldt for the Hoopa School Readiness Program to clarify expectations in the Scope of Work and increase funding for fiscal year 2010-11; Authorize the Chair to sign three originals of the Agreement; direct the Clerk of the Board to return the three signed copies of the Agreement to the Public Health Branch; and direct the Auditor-Controller to supplement the budget in the amount of $4,305 in Fund 1175, Budget Unit #420, MCAH Coordination, for Fiscal Year 2010-11 per attachment C.


Health and Human Services - Social Services Branch

13.               Approval of Agreement with Redwood Community Action Agency to provide Housing and CalWORKs Welfare-to-Work Program Employment services at the Multiple Assistance Center.

RECOMMENDATION: That the Board of Supervisors approve Agreement with Redwood Community Action Agency to provide housing at the Multiple Assistance Center and CalWORKs Welfare-to-Work Program Employment services to families referred from the Social Services Branch; authorize the Chair to execute three copies of the Agreement and direct the Clerk of the Board to route two fully executed copies of the Agreement to the Director, Department of Health and Human Services Social Services Branch.

14.               Agreement with the North Coast Rape Crisis Team for the Children's Trust Fund grant Program.

RECOMMENDATION: That the Board of Supervisors award the Children's Trust Fund Agreement to the North Coast Rape Crisis Team for fiscal years 2011/12, 2012/13, and 2013/14; Approve Agreement with the North Coast Rape Crisis Team in the amount of $10,000 annually for the next three fiscal years 2011/12, 2012/13, and 2013/14; authorize the Chairperson to execute three copies of the Agreement; and direct the Clerk of the Board to route two copies of the executed Agreement to the Director of the Social Services Branch.

15.               Agreement between Humboldt County Department of Health and Human Services - Social Services Branch and Remi Vista, Inc. for the continuation of the Transitional Housing Placement Program (THPP).

RECOMMENDATION: That the Board of Supervisors approve the proposed agreement for fiscal year 2011-2012 between the County of Humboldt and Remi Vista, Incorporated for the Transitional Housing Placement Program; Authorize the Chair of the Board to execute three copies of the agreement; direct the Clerk of the Board to route to fully executed copies of the Agreement to the Director, DHHS - SSB.

16.               Approve Amendment to Agreement with CareAccess for web portal services of the Aging and Adult Client Tracking System (AACTS) used by the Adult Protective Services (APS) program.

RECOMMENDATION: That the Board of Supervisors approve Amendment to Agreement with CareAccess for fiscal year 2011/2012; direct the Clerk of the Board to execute three copies of the Amendment to Agreement; and direct the Clerk of the Board to route the three copies of the Amendment to Agreement to the Director of the Department of Health and Human Services, Social Services Branch.

17.               Approve Agreement between the Humboldt County Department of Health and human Services and the Redwood Community Action Center for the Transitional Housing Program - Plus (THP-Plus).

RECOMMENDATION: That the Board of Supervisors approve the Agreement between County of Humboldt and Redwood Community Action Agency Youth Services Bureau, to provide Transitional Housing Program - Plus Services for emancipated former foster youths; authorize the Chair to executed three copies of the Agreement; and direct the Clerk of the Board to return two fully executed copies of the Agreement to the Director of the Department of Health and Human Services.


18.               Agreement between the Humboldt County Department of Health and Human Services and the McKinleyville Community Collaborative fro the Child Abuse Prevention, Intervention, and Treatment Program (CAPIT).

RECOMMENDATION: That the Board of Supervisors award the Child Abuse Prevention, Intervention and Treatment program (CAPIT) Agreement to McKinleyville Community Collaborative for fiscal years 2011/12, 2012/13, and 2013/14; approve Agreement with McKinleyville Community Collaborative in the amount of $75,000 annually for each of the next three fiscal years 2011/12, 2012/13, and 2013/14; authorize the Chairperson to execute three copies of the Agreement; and direct the Clerk of the Board to route two copies of the executed Agreement to the Director of the Social Services Branch.

Public Works

19.               Adoption of Mitigated Negative Declaration of Environmental Impact and Approval of Plans and Specifications for Bald hills Road Improvement Project (F4R300), P.M. 10.1 to 13.4, Contract No.: 321601.

RECOMMENDATION: That the Board of Supervisors review the attached Initial Study and mitigated Negative Declaration of Environmental Impact for Bald Hills Road Improvement Project, P.M. 10.1 to 13.4; approve the recommended mitigation measures; incorporate the approved mitigation measures as part of the project; and approve and adopt the recommended mitigation monitoring and discovery plan; find that the procedural requirements of the California Environmental Quality Act have been satisfied for this project; find, based on the evidence presented in the staff report, that the project, with incorporation of the mitigation measures, will not have a significant effect on the environment; approve the attached mitigated Negative Declaration prepared by staff; direct the Public Works to file a Notice of Determination with the County Clerk (Recorder's Office) review and approve the plans and specifications for the above-mentioned project as required by Section 22037 of the California Public Contract Code, with bids to be opened on Tuesday, July 19, 2011, at 2:00 PM.

20.               Pay Differential for Hank Seemann, Environmental Services Manager.

RECOMMENDATION: That the Board of Supervisors approve the continuation of the temporary increase in compensation of 10 salary ranges for Hank Seemann, Environmental Services Manager, from July 3, 2011 and continuing through December 24, 2011 or until a classification review of Mr. Seemann's position is completed, whichever occurs first, pursuant to Section 7 of the Humboldt County Salary Resolution.

21.               Transportation Development Act Revised Claim.

RECOMMENDATION: That the Board of Supervisors approve the attached Transportation Development Act (TDA) Revised Claim for FY 2010/11 in the amount of $2,037,280.82 and direct the Clerk of the Board to forward the revised claim to the Humboldt County Association of Governments (HCAOG).

22.               Big Lagoon County Park Enhancement Project.

RECOMMENDATION: That the Board of Supervisors adopt the attached resolution approving Grant Agreement No. 10-117 with the State Coastal Conservancy for preparation of preliminary designs for the Big Lagoon County Pak Enhancement Project and authorizing the Director of Public Works of his designee to sign the grant agreement; and direct the County Administrative Office to incorporate the attached revenue and expenditure items in the final budget for FY 2011-12.

23.               Proposed Naming of Newly Constructed Bridge of Indian Creek on Mattole Road as "Miss Katie" Cummings Memorial Bridge.

RECOMMENDATION: That the Board of Supervisors approve the proposed naming of the newly constructed bridge over Indian Creek on Mattole Road as the "Miss Katie" Cummings Memorial Bridge.

Sheriff

24.               Application for Reimbursement Under the State Criminal Alien Assistance Program.

RECOMMENDATION: That the Board of Supervisors authorize the Sheriff's Office's application to the State Criminal Alien Assistance Program (SCAAP) for reimbursement by accepting all assurances and certifications contained in the on-line application.

County Administrative Office

1.                   County Administrative Officer Report: State Budget and Various Other Topics.

RECOMMENDATION: That the Board of Supervisors receive an oral report from the County Administrative Officer on the latest information regarding the State budget process and various other topics, and take action as may be required.

2.                   Response to the 2010-11 Grand Jury Report 2011-AF-2.

RECOMMENDATION: That the Board of Supervisors review and modify the proposed response from the Board of Supervisors to Grand Jury Report 2011-AF-2; review and modify the proposed Grand Jury responses from departments under the appointment authority of the Board of Supervisors; approve the documents, as may be modified, as the response; direct the Clerk of the Board, within five working days, to submit the final response with an accompanying Board Order, to the Presiding Judge of the Superior Court; and direct the Clerk of the Board, within five working days, to submit two copies of all responses to the County Clerk / Recorder, one of which will be forwarded to the State Archivist, together with a copy of the original report.

Personnel

3.                   Exception to the Hiring Freeze for on Personnel Analyst I/II and on Administrative Services Officer Position Unit 130 (Personnel Department).

RECOMMENDATION: That the Board of Supervisors approve an exception to the hiring freeze for 1.0 Personnel Analyst I/II (salary range 404/432, class 0684, position in Budget Unit 130 with a start date not prior to August 20, 2011; and approve an exception to the hiring freeze for 1.0 Administrative Services Officer (salary range 409, class 0776, position 01) position in Budget Unit 103 with a start date of July 1, 2011.

Community Development Services

1.                   Submission of Final Studies to the California Department of Housing and Community Development, Community Development Block Grant; and Weott Broadband Project.

RECOMMENDATION: That the Board of Supervisors receive reports funded by the State of California Community Development Block Grant (CDBG) Number 08-PTAE-5417 a.) Orick Hydraulic Evaluation Report for Levee Certification b) Orick Levee Accreditation and Summary Report c) Weott Broadband Project; open the public hearing and receive public testimony; close the public hearing, and direct Staff to submit final closing documents to the State and draw the final 10% holdback funds.

Elections

2.                   Public Hearing on Redistricting and Appointment of Committee.

RECOMMENDATION: That the Board of Supervisors open the public hearing and receive public testimony on redistricting; close the public hearing and appoint a committee consisting of Sheriff Mike Downey, District Attorney Paul Gallegos, Assessor Mari Wilson and Registrar of Voters Carolyn Crnich to study all issues relevant to redistricting as described in Section 21500 of the California Elections Code and submit a plan for redistricting that meets California Law to the Board of Supervisors for approval; direct that at least one public hearing be scheduled in each of the five districts; and direct the designated committee to return to your Board with their recommendations for redistricting on August 16, 2011.
