Auditor-Controller

1.                   Payroll Clearing Appropriation Transfer for Fiscal Year 2010-2011.

RECOMMENDATION: That the Board of Supervisors approve the attached fiscal year 2010-2011 appropriation transfer fro $1,255,896; and authorize the Auditor-Controller to make minor budget adjustments as needed to provide sufficient appropriations to cover actual expenditures for fiscal year 2010-2011.


Health and Human Services - Mental Health Branch

2.                   First Amendment to Lease for 2933 H Street Modular Building.

RECOMMENDATION: That the Board of Supervisors approve and authorize the Chair to execute the First Amendment to Lease with Performance Modular, Inc. for the modular building located at 2933 H Street, Eureka.

Health and Human Services - Public Health Branch

3.                   Memorandum of Agreement with North Coast Clinics Network for the EpiCenter Dental Module.

RECOMMENDATION: That the Board of Supervisors approve and authorize the Public Health Branch Director to sign the Memorandum of Agreement with North Coast Clinics Network (NCCN) in the amount of $2,000 in Fund 1175, Budget Unit 420 - Maternal, Child, Adolescent Health; and approve the supplemental budget and direct the Auditor-Controller to supplement Fund 1175, Budget Unit 420 - MCAH per attachment A for Fiscal Year 2011-12.

4.                   State Standard Agreement with California Department of Public Health for the Local Incentive Award in the Nutrition and Physical Activity Program for the period of October 1, 2011 through September 30, 2012.

RECOMMENDATION: That the Board of Supervisors approve and execute State Standard Agreement number 11-10207 with the California Department of Public Health (CDHP) for the Local Incentive Award for up to $250,000 for the period of October 1, 2011 through September 30, 2012; authorize the Chair to sign two originals of the Agreement, two originals of the Lobbying Disclosures and one of the Certification Clauses; authorize the Public Health Branch Director to sign any subsequent amendments directly related to the Agreement; direct the Clerk the of the Board to return all signed originals of the Agreements of the Agreement to the Public Health Branch; direct the Auditor-Controller to adjust Fund 1175, Budget Units 433 - Nutrition for Fiscal Year 2011-12 per Attachment C.

Library Services

5.                   Lease Agreement for the Willow Creek Branch Site.

RECOMMENDATION: That the Board of Supervisors approve terminating the current lease with A.K. Tonkin and Lillian Tonkin; authorize Real Property to send a letter of termination to A.K. Tonkin and Lillian Tonkin; approve and authorize the Chairperson to execute the attached lease with Roger and Nancy Brown, DBA Brown Rental; and return one executed lease to Real Property for transmittal to Lessor.

Personnel

6.                   Revised Class Description for Health Officer.

RECOMMENDATION: That the Board of Supervisors approve the revised class description for Health Officer.

Public Works

7.                   Construction Storm Damage Repairs on Alderpoint Road (F6B165) PM 45.40-45.46 & PM 45.64-45.65, Project No.: ER-4400(057) & ER-4400(056), Contract No.: 206382 & 206385.

RECOMMENDATION: That the Board of Supervisors authorize the Director of Public Works to sign Contract Change Order Number 1 for the total amount of $48,482.34 and Contract Change Order Number 2 for the total amount of $5,302.46; approve the attached Appropriation Transfer to provide funding.


8.                   Construction of School Road (C3L070) Sidewalk and Realignment Project (Phase I) Between Bugenig Avenue and Highway 101 (pm 0.37 to 0369); Contract No.: 209801.

RECOMMENDATION: That the Board of Supervisors authorize the Director of Public Works to sign Contract Change Order Number 1 for the total amount of $146,913.94 and approve the attached Appropriation Transfer to provide funding.

9.                   Construction of Storm Damage Repairs on Mattole Road (3C010) PM 6.60, Project No.: ER-4400(060), Contract No.: 206394.

RECOMMENDATION: That the Board of Supervisors receive and file the bids for Construction of Storm Damage repairs on Mattole Road at PM 6.60; award the contract to G.R. Wilcox Enterprises, Inc. for the total amount of $51,168.80; authorize the Chairman to execute the project agreement upon submittal of the appropriate bonds and insurance; and direct the Auditor-Controller to establish a new contract account 1200321-0273 and transfer $53,727 to the account per the attached appropriation transfer.

Sheriff

10.               Authorize the Chair and the Sheriff to sign Documents Necessary to Submit a Non-Competitive Application for the Anti-Drug Abuse (ADA) Enforcement Team Program.

RECOMMENDATION: That the Board of Supervisors authorize the Chair to sign the Authorization for the Certification of Assurance of Compliance for the none competitive Anti-Drug Abuse Enforcement Team Program application; and authorize the Sheriff to sign the Certification of Assurances, face sheet and other grant documents as Authorized Agent.

D.                 SPECIAL PRESENTATIONS

1.                   Proclaim October 20, 2011 as Credit Union Day in Humboldt County. (Supervisor Smith)

RECOMMENDATION: Adopt the Proclamation.

E.                  ITEMS PULLED FROM CONSENT (IF APPLICABLE)

F.                  MATTERS INITIATED BY BOARD MEMBERS

1.                   Appointment of 2011/12 Member and Alternate to the California State Association of Counties Board of Directors. (Supervisor Mark Lovelace)

RECOMMENDATION: Make appointments.

2.                   Standing Item--Reports on Board Member Travel at County Cost, Meeting Attendance, and Supervisorial District Matters.

G.                 DEPARTMENT REPORTS

County Administrative Office

1.                   County Administrative Officer Report:  County Hold Music and Various Other Topics.

RECOMMENDATION: That the Board of Supervisors receive an oral report from the County Administrative Officer on hold music for the County telephone system, and various other topics and take action as needed.

Community Development Services

2.                   Local Strategic Plan for the Humboldt County Local Workforce Investment Area.

RECOMMENDATION: That the Board of Supervisors receive staff report and review Workforce Investment Area Local Strategic Plan (the Plan).  Approve Plan modification, direct Chair of the Board to sign the Plan, and authorize Staff to submit Plan modification to State.

1.                   It is the intention of the Board of Supervisors to meet in closed session to consider the Public Employee Performance Evaluation for the position of Director of Public Works pursuant to Government Code Sections 54954.5 and 54957.

Community Development Services

1.                   Public Hearing to discuss adopting a revised Community Development Block Grant Program Income Reuse Plan.

RECOMMENDATION: That the Board of Supervisors receive staff report; open the public hearing and receive public testimony, close the public hearing, and adopt the attached Resolution amending County of Humboldt Community Development Block Grant (CDBG) Program Income Reuse Plan.


Elections

2.                   Public Hearing to Consider Proposed Ordinance Relating to Boundary Changes to Supervisorial Districts.

RECOMMENDATION: That the Board of Supervisors open the public hearing; hear the report on supervisorial redistricting and receive public comment; introduce by Title, Ordinance No. ____, amending Chapter 2 of Division 2 of Title II of the Humboldt County Code relating to boundary changes to supervisorial districts, and waive further reading of the ordinance; close the public hearing; approve the new adjusted supervisorial district boundaries as set forth in the proposed ordinance; set the proposed ordinance for adoption on October 25, 2011; direct the Clerk of the Board to post a certified copy of the full text of the proposed ordinance in the office of the Clerk of the Board at least five days prior to the Board meeting at which the ordinance will be adopted and to post and publish in a newspaper of general circulation in the County of Humboldt a pre-adoption summary of the ordinance at least five days before the date set for adoption; direct the Clerk of the Board to publish a post-adoption summary of the ordinance within 15 days after adoption of the ordinance; direct staff to prepare maps that reflect the legal descriptions of the new supervisorial district boundaries as set forth in the proposed ordinance and delegate to the Chair the dating and approval of the final maps upon approval of the County Surveyor and direct the Clerk to file the approved maps with the County Surveyor; and direct the Clerk of the Board to file with the County Surveyor a certified copy of said ordinance showing the date of adoption.
