County Administrative Office

12.               Memorandum of Understanding (MOU) between the County of Humboldt and Salmon Creek Volunteer Fire Department for the provision of Workers' Compensation Coverage.

RECOMMENDATION: That the Board of Supervisors approve the attached Resolution to add Salmon Creek Volunteer Fire Department to the County's primary and excess Workers' Compensation policies as a covered party/named insured; and authorize the Chair to sign the attached MOU to add Salmon Creek Volunteer Fire Department to the County's Workers' Compensation effective July 1, 2012.

13.               Operating Agreement with Humboldt Lodging Alliance.

RECOMMENDATION: That the Board of Supervisors approve the attached Agreement for Services between the County of Humboldt and the Humboldt Lodging Alliance for operation of the Humboldt County Tourism Business Improvement District (HCTBID), effective July 1, 2012; authorize the Chair to sign two copies of the Agreement; and direct the Clerk of the Board to return one copy to the Humboldt Lodging Alliance.

County Counsel

14.               Special Assessments for Unpaid Administrative Penalties - Gyle Maruska; Redwood Valley, AP# 316-171-019.

RECOMMENDATION: That the Board of Supervisors, pursuant to Humboldt County Code Section 2131-8(e): adopt the attached resolution declaring a special assessment against real property for an unpaid administrative penalty.

Health and Human Services - Mental Health Branch

15.               Agreements Between Changing Tides and Family Services and Humboldt County for the Provision of Mental Health Services and for the Provision of Therapeutic Behavioral Services for Fiscal Years 2012-13 and 2013-14.

RECOMMENDATION: That the Board of Supervisors: Approve and authorize the Chair to sign three (3) copies of the Agreement with Changing Tides Family Services for the provision of Mental Health Services (Attachment 1) for Fiscal Years 2012-13 and 2013-14; Approve and authorize the Chair to sign three (3) copies of the Agreement with Changing Tides Family Services for the provision of Therapeutic Behavioral Services (Attachment 2) for Fiscal Years 2012-13 and 2013-14; and Direct the Clerk of the Board to return two (2) executed copies of each Agreement to the Department of Health and Human Services (DHHS) - Social Services Administration.

16.               Agreement Between Humboldt County and Double RR Care Home Corporation, Fiscal Year 2012-2013.

RECOMMENDATION: That the Board of Supervisors: Approve the Agreement with Double RR Care Home Corporation, Fiscal Year 2012-2013; and Authorize the Chair of the Board to execute three (3) copies of the Agreement effective July 1, 2012; and Direct the Clerk of the Board to return two (2) signed originals of this Agreement to Mental Health Branch Administration.

17.               Agreement with St. Joseph Hospital of Eureka for Fiscal Years 2012-2014.

RECOMMENDATION: That the Board of Supervisors: Approve and authorize the Chair to sign the Agreement with St. Joseph Hospital Eureka for hospital medical services for Fiscal Years 2012-2014; and Direct the Clerk of the Board to return two signed copies of the Agreement to the Mental Health Branch.

 

 

 

 

 

18.               Approval of Agreement between Humboldt County and North Coast Substance Abuse Council, Inc. for Residential Services for Fiscal Year 2012-2013.

RECOMMENDATION: That the Board of Supervisors: Approve the Agreement between County of Humboldt and North Coast Substance Abuse Council, Inc. for Fiscal Year 2012-2013; and Authorize the Chair to execute three (3) copies of the Agreement effective July 1, 2012; and Direct the Clerk of the Board to return two (2) copies of the executed agreement to the Department of Health and Human Services, Mental Health Branch Administration.

19.               Approval of Agreement between Humboldt County and Alcohol Drug Care Services, Inc. for Residential and Detoxification Services for Fiscal Year 2012-13.

RECOMMENDATION: That the Board of Supervisors: Approve the Agreement between Humboldt County and Alcohol Drug Care Services, Inc. for Fiscal Year 2012-2013; and Authorize the Chair of the Board to execute three (3) copies of the Agreement effective July 1, 2012; and Direct the Clerk of the Board to return two (2) copies of the Agreement to Mental Health Branch Administration.

Health and Human Services - Public Health Branch

20.               Amendment to Agreement #09-86017 for Medi-Cal Administrative Activities with the California Department of Health Care Services.

RECOMMENDATION: That the Board of Supervisors approve and direct the Chair to sign four (4) original signature pages for Amendment A01 to Medi-Cal Administrative Activities (MAA) Agreement #09-86017 between the County of Humboldt and the California Department of Health Services (CDHS) and sign one (1) Contractor Certification Clauses (Exhibit A); and Direct the Clerk of the Board to return the four (4) signed signature pages and one (1) signed Contract Certification Clauses to the Public Health Branch for submission to the State.

21.               Approve the State Standard Agreement with the State Water Resources Control Board for $368,724 in Fund 1175, Budget Unit 432, Local Oversight Program for fiscal year 2012-13.

RECOMMENDATION: That the Board of Supervisors: Approve State Standard Agreement 12-003-0250, with the State Water Resources Control Board, in the amount of $368,724 in Fund 1175, Budget Unit 432, Local Oversight Program for fiscal year 2012-13; Direct the County Administrative Office to incorporate the attached Supplemental Budget for Fund 1175, Budget Unit 432, Local Oversight Program, into the final adopted budget for fiscal year 2012-13; Authorize the Chair to sign three (3) originals of the Agreement and one (1) Contractor Certification Clauses (CCC-307); and Direct the Clerk of the Board to return the agreement, three original signature pages and Contractor Certification Clauses to the Environmental Health Division for submission to the State Water Resources Board.

Health and Human Services - Social Services Branch

22.               Grant Agreements for Services with Community /Family Resource Centers for CalWorks Outreach and Support and CalFresh Outreach and Support for Fiscal Year 2012/2013.

RECOMMENDATION: That the Board of Supervisors approve Grant Agreements, totaling $400,000 with the Community/Family Resource Centers participating with CalWORKs Outreach and Support and CalFresh Outreach and Support for fiscal year 2012/2013; and authorize the director of the Social Services Branch of the Department of Health and Human Services (DHHS), to execute these Grant Agreements substantially in the form of the attached sample copy of the Grant Agreement upon proof of insurance.

 

 

 

23.               Approval of Services Agreement with Redwood Community Action Agency (RCAA) for the Provision of Mental Health Services to Medi-Cal Beneficiaries under 21 Years of Age for Fiscal Years 2012-12 and 2013-14.

RECOMMENDATION: That the Board of Supervisors Approve and authorize the Chair to sign three (3) copies of the Agreement with RCAA for the Provision of Mental Health Services to Medi-Cal Beneficiaries under 21 years of Age (Attachment 1) for Fiscal Years 2012-13 and 2013-14; and Direct the Clerk of the Board to return two (2) signed copies of the Agreement to the Department of Health and Human Services (DHHS) Social Services Administration.

24.               Approval and Execution of UC Davis Training Contracts for Eligibility Services and Child Welfare Service Training.

RECOMMENDATION: That the Board of Supervisors: Approve and authorize the Chair to sign the attached contracts in the amounts of $108,680.00 to provide Eligibility Services training and for the amount of $29,260.00 for Child Welfare training with the University of California - Davis (US-Davis), for the provision of training to Department of Health and Human Services - Social Services Branch (DHHS-SSB) staff for FY 2012-2013; Grant a waiver of the Nuclear Free Ordinance; and Direct the Clerk of the Board to return two (2) executed copies of each contract to the DHHS-SSB Director.

25.               Approval of the Memorandum of Understanding for continued Third Party Access to the California Department of Child Support Enforcement Program.

RECOMMENDATION: That the Board of Supervisors:  Approve the attached Memorandum of Understanding (MOU) for continued Third Party Access to the California Department of Child Support Services Child Support Enforcement Program; and Authorize the Director of the Department of Health and Human Services Social Services Branch to sign three (3) copies of the Memorandum of Understanding.

26.               Agreements between Remi Vista, Inc. and Humboldt County for the provision of Mental Health Services and for the provision of Therapeutic Behavioral Services for Fiscal Years 2012-13 and 2013-14.

RECOMMENDATION: That the Board of Supervisors approve and authorize the Chair to sign three (3) copies of the Agreement with Remi Vista, Inc. for the provision of Mental Health Services for Fiscal Years 2012-13 and 2013-14; approve and authorize the Chair to sign three (3) copies of the Agreement with Remi Vista, Inc. for the provision of Therapeutic Behavioral Services for Fiscal Years 2012-13 and 2013-14; and Direct the Clerk of the Board to return two (2) executed copies of each Agreement to the Department of Health and Human Services (DHHS) - Social Services Administration.

Library Services

27.               Change of Humboldt County Library Bookmobile location in Orleans, CA.

RECOMMENDATION: That the Board of Supervisors approves a change of the Humboldt County Library bookmobile stop in Orleans, Ca effective July 1, 2012.

Personnel

28.               Phase I in the Implementation of the Human Resources Department.

RECOMMENDATION: That the Board of Supervisors adopt the new department head level job class of Director of Human Resources (class 0821, salary $10,834 monthly, unit 10) into the classification plan, effective immediately; adopt Resolution No. ____ approving the Amendment to the Compensation Plans for Management and Confidential Employees and Elected and Appointed Department Heads for fiscal yeas 2011-2012 and 2012-2013, effective June 24, 2012; approve the reallocation of the Personnel Director (class 0822, salary $10,079 monthly, unit 10) position in budge unit 130, currently occupied by Dan Fulks, to Director of Human Resources (class 0821, salary $10,834 monthly, unit 10) in budget unit 130, effective June 24, 2012; adopt Resolution No.______ specifying references to Risk Manager and Risk Management in past Board actions to the Director of Human Resources, effective June 24, 2012; and delete the department head level job class of Personnel Director (class 0822, salary $10,079 monthly, unit 10) from the classification plan, effective June 24, 2012.

Public Works

29.               Aviation Surplus Vehicle

RECOMMENDATION: That the Board of Supervisors declare the following vehicle surplus: 1996 Ford F150 , Vehicle #10654, 118,000 miles; and authorize the Purchasing Agent to dispose of the surplus vehicle and deposit the revenue from the sale in the Aviation Enterprise Fund.

30.               Federal Apportionment Exchange and State Match Program for Regional Surface Transportation Program (RSTP) Funds.

RECOMMENDATION: That the Board of Supervisors authorize the Chair to sign two copies of the Agreement No. X12-5904(125); and direct the Clerk of the Board to return two signed copies of the agreement and the authorizing Board Order to the Department of Public Works for transmittal to California Department of Transportation (Caltrans).

31.               Second Amendment to Agreement for Airport Construction Engineering Services Runway 01 Safety Area for the Airport Improvement Program (AIP) Grant 03-06-0010-34/35, Project 919216.

RECOMMENDATION: That the Board of Supervisors authorize the Chair to execute all four (4) copies of the attached Second Amendment to the Construction Engineering Services Agreement between the County and Mead & Hunt, Inc., Santa Rosa, California, for the construction engineering services for the Arcata-Eureka Airport (ACV) Runway 01 Safety Area Grading improvements; and direct the Clerk of the Board to file one (1) executed original, and return the remaining amendments to Public Works.

32.               Summary Vacation of a One Foot Non-Vehicular Access Easement, Along Brannon Mountain Road, APN 522-211-051.

RECOMMENDATION: That the Board of Supervisors adopt the attached Resolution summary vacating an existing one foot non-vehicular access easement along to the Brannon Mountain Road; and direct the Clerk of the Board to return a certified copy of the Resolution summarily vacating the one foot non-vehicular access easement to the Land Use Division for recordation.

33.               Supplemental Budget for Capital Projects and Bid Authorization for Public for Public Health Furnace Replacement Project, County of Humboldt, Project No. 162312.

RECOMMENDATION: That the Board of Supervisors direct the County Administrative Office to incorporate the attached Supplemental Budget Fund 1175, Budget Unit 400 Public Health Administration into the final adopted budget for FY 2012-13; receive the plans and specifications, authorize the Clerk of the Board to advertise the above-mentioned project, pursuant to Section 22073 of the California Contract Code, with bids to be opened on Tuesday, July 10, 2012 at 2:00.

RECOMMENDATION: Adopt the Proclamation.

County Administrative Office

1.                   Responses to 2011-12 Grand Jury Report.

RECOMMENDATION: That the Board of Supervisors review and modify as necessary the proposed response from the Board of Supervisors to Grand Jury Report 2012-Admin&Finance-01; review and modify as necessary the proposed Grand Jury responses from departments under the appointment authority of the Board of Supervisors; approve the documents, as may be modified, as the response; direct the Clerk of the Board within five working days, to submit the final response with an accompanying Board Order, to the Presiding Judge of the Superior Court; and direct the Clerk of the Board , within five working days, to submit two copies of all responses to the County Clerk /Recorder, one of which will be forwarded to the State Archivist, together with a copy of the original report.

Planning and Building

2.                   Introduction of the Planning Commission Approve Draft General Plan and Supporting Documents and Discussion of Board Review Process for Future Public Hearings.

RECOMMENDATION: That the Board of Supervisors receive transmittal of the Planning Commission approved Draft General Plan and supporting documents; receive staff's introduction of the Draft General Plan and description of the proposed public hearing review process for the future meetings; receive public input; deliberate as necessary regarding the Board's review process for the Draft General Plan and continue the item to June 18, 2012 for the first public hearing.

PUBLIC WORKS

1.                   Formation of Permanent Road Division (PRD) No. 1000, Zone No. 007 and Imposition of Parcel Charges for Central Estates Phase 2A Subdivision, McKinleyville.

RECOMMENDATION: That the Board of Supervisors open the public hearing regarding the formation of Permanent Road Division No. 1000, Zone No. 007 and imposition of parcel charges for road maintenance for the Central Estates Phase 2A Subdivision, receive public testimony, and close the public hearing; direct the Auditor-Controller to tally the ballots received to impose parcel charges for road maintenance in the Central Estates Phase 2A Subdivision; based upon the public testimony and results of the election, adopt the attached Resolution to Establish Humboldt County Permanent Road division 1000, Zone No. 007 and to Impose Parcel Charges for Road Maintenance; adopt the Resolution of Acceptance to accept the rights of way for Elmwood Place for public road purposes; and Parcel H for pedestrian easement purposes, both as shown dedicated to the County of Humboldt on Tract No. 641, filed in Book 25 of Maps, pages 30 and 31, in the Office of the Humboldt County Recorder; approve the establishment of an interest-bearing fund for deposit of finds received from parcel assessments and direct the Auditor-Controller to set up a fund entitled Permanent Road Division No. 1000, Zone 007 with fund number 2307; and direct the Auditor-Controller to calculate yearly the compounded inflation factor based upon the Consumer Price Index (CPI) to the parcel charges for road maintenance.

RECOMMENDATION: That the Board of Supervisors receive an oral report from the County Administrative Officer on his attendance at the Forest Counties School Coalition meeting in Sacramento, and various other topics, and take action as may be required.

2.                   Standing Item--Reports on Board Member Travel at County Cost, Meeting Attendance, and Supervisorial District Matters.
