1.                   Request to Direct the Agricultural Commissioner to Avoid an Anticipated Overdraft for Contracted Animal Shelter Services with the Sequoia Humane Society

RECOMMENDATION: Approve request, and approve the requested transfer of $42,910 from Contingency Fund to Budget Unit 278, Account No. 1100 -278-2-118

2.                   Resolution Authorizing Humboldt County to be a Contributing Signatory to a Memorandum of Understanding for a Coordinated Approach to Restoring Fish Passage for Anadromous Salmonids in Coastal Watersheds (Supervisor Smith)

RECOMMENDATION: Adopt Resolution No 03-__.

3.                   Letter of Appreciation to John Frederick for Having Served on the Humboldt County Fish and Game Advisory Commission (Supervisor Geist)

RECOMMENDATION: Authorize Chair to sign letter of appreciation.

4.                   Resolution Declaring August 30, 2003, as Bigfoot Days in Willow Creek (Supervisor Geist)

RECOMMENDATION: Grant exemption for Staff Services Analyst II position in Child Support Services.

10.               Agreement with the State of California Employment Development Department in Order to Secure Wage and Employer Information for the Purpose of Collecting Restitution, Fines, Penalties, Assessments, and Fees Owed to the County or State as Directed by State Mandate or Court Order

RECOMMENDATION: Authorize County Administrative Office to sign the agreement.

District Attorney

11.               Victim Witness Assistance Program; Grant Application for Fiscal Year 2003-04; State Office of Criminal Justice Planning

RECOMMENDATION: Authorize Chair to execute original Grant Proposal/Award Face Sheet and Certificate of Assurance of Compliance, which will allow continuation of services to County victims of crime for another fiscal year.

General Services

12.               Lease for Public Works Sign Shop and Vehicle Storage

RECOMMENDATION: Approve lease with Charlene and Ron Lundblade for property located at 112 W. Third Street in Eureka.

Health and Human Services - Mental Health Branch

13.               Agreement with Michael Burleson, M.D. for Non-Emergency Medical Services for Fiscal Years 2002-2003 and 2003-2004

RECOMMENDATION: Approve agreement.

14.               Agreement with Cloney's Prescription Pharmacy for Consultation and Medication Services--Fiscal Year 2002-2003

RECOMMENDATION: Ratify agreement.

Health and Human Services - Public Health Branch

15.               Amendment No. A-2 to Agreement No. 00-90745 with the California Department of Health Services--Domestic Violence Prevention Program

RECOMMENDATION: Approve amendment, extending the term of the grant through June 30, 2005, with an additional $150,000 in funding each year for Fiscal Years 2003 and 2004.

16.               Expenditure Report and Certification Against Supplanting--Progress Report to the State of California Department of Health Services; Local Public Health Preparedness and Response to Bioterrorism Program

RECOMMENDATION: Receive, approve, and sign report.

17.               Revised Allocation--Deputy Health Officer Position

RECOMMENDATION: Approve revised allocation.

18.               Temporary Increase in Pay for Jeri Scardina, Social Worker Supervisor II; and Continuing for Six Months, or Until the Vacant Program Manager II Position is Filled (Whichever Comes First)

RECOMMENDATION: Approve temporary increase in pay.


19.               Two Years' Added Service Credit--Public Employees' Retirement System

RECOMMENDATION: That the Board give notice of its intent to adopt a resolution to grant another designated period for two years' additional service credit, for the classifications of Senior Investigator (D.A.); Facility Maintenance Superintendent, Computer Operations Supervisor, Senior Systems & Applications Programmer, Deputy Director--Social Services, Mental Health Program Manager, Vocational Counselor I/II (Employment Training Division); Staff Services Manager I (Fiscal), and Department Information Systems Manager.


20.               Request for an Increase in the Fees for Conservatorship Investigations and Written Report to the Court from $200 to $324.50

RECOMMENDATION: Approve request by adopting Resolution No. 03-__.

Public Works

21.               Resolution to Amend Resolution No. 90-123 to Summarily Vacate a Portion of Beach Road (One-Foot-Wide Non-Vehicular Access Strip); Shelter Cove Subdivision

RECOMMENDATION: Adopt Resolution No. 03-__, which corrects an error discovered on a title search on a parcel of land in said subdivision.

22.               Acquisition of Right of Way for Chartin Road; Blue Lake

RECOMMENDATION: Accept three Easement Deeds from the Blue Lake Rancheria of California for the improvement of Chartin Road.

23.               Supplemental Budget for Land Use Division ($35,000)

RECOMMENDATION: Adopt supplemental budget to provide funding for Road Fund staff that worked on subdivision projects.



24.               License Agreement for Geoff D. Werronen, d.b.a. Northwind Delivery Service, to Provide Ground Courier Service from County Airports

RECOMMENDATION: Approve license agreement.

25.               Overflight Easement to the County of Humboldt Executed by Gary and Christine Manhart for the Benefit of the Arcata-Eureka Airport

RECOMMENDATION: Accept Overflight Easement.



26.               License Agreement for Alfonse E. Castaldi, d.b.a. Executive Air Charter, to Provide Air Charter Service from Murray Field Airport

RECOMMENDATION: Approve license agreement.

27.               JLF Construction Final Map Subdivision (Bates Lane); APN 510-111-57, McKinleyville Area

RECOMMENDATION: Authorize release of Security for Faithful Performance ($25,135) and Payment Security to Contractors ($12,570).

28.               Storm Damage Repairs to Mattole Road; Contract No. 971072

RECOMMENDATION: Approve plans and specifications, and hold bid opening on July 29, 2003.




29.               Request for Waiver of Section 2,D(6) of Salary Resolution 82-170 for Extra-Help Legal Office Assistant Heather Benson

RECOMMENDATION: Approve request, and grant an extension of her temporary appointment from 960 hours to 1068 hours.

30.               Renewal of Joint Operating and Financial Plan for Controlled Substances from United States Department of Agriculture, Forest Service, for 2003

RECOMMENDATION: Approve Agreement No. 02-LE-11051360-052 in support of operations to suppress manufacturing and trafficking of controlled substances on or affecting the administration of National Forest System lands, wherein the Forest Service agrees to reimburse the County for expenditures associated with detection, surveillance, and eradication of activities associated with illegal production or trafficking of controlled substances.

31.               Request to Accept, on Behalf of the Sheriff's Office, Two TREK Bicycles Equipped for Law Enforcement

RECOMMENDATION: Accept bicycles purchased by Deputy Sheriff Greg Bickel in memory of his mother, Jeane-Mance Bickel., for use at the McKinleyville Station.
