RECOMMENDATION: Appoint Susan Fregeau, Dawn Watkins, and Larry Alexander to said Board.

8.                   Second Amendment to Service Agreement With Crestwood Behavioral Health,  Inc. For Full-day Rehabilitation Services to Authorized Medi-Cal-Eligible Beneficiaries to Increase Contract Value for Fiscal Year 2005-06 to $807,000 Per Fiscal Year

RECOMMENDATION: Authorize Chair to execute second amendment.

9.                   Increase in Compensation for Physician/Psychiatrists and Medical Director to Reflect Salary Increase for Humboldt County Employees

RECOMMENDATION: Authorize the Director of Health and Human Services to sign amendments to the Physician/Psychiatrist Employment Agreements for Dr. Orm Aniline, Dr. David Aryanpur, Dr. Stephen Williams, Dr. Chan Nguyen, and Dr. Paula Edwalds to reflect salary increases from Range 612 to Range 614 effective June 4, 2006, and from Range 614 to range 618 effective July 2, 2006, and from Range 618 to Range 620 effective July 8, 2007 and to sign an amendment to the Physician/Psychiatrist Employment Agreement for Medical Director, Dr. Kate Bell, to reflect salary increase from Range 622 to Range 624 effective June 4, 2006 and from Range 624 to Range 628 effective July 2, 2006 and from Range 628 to Range 630 effective July 8, 2007.

10.               Memorandum of Agreement (MOA) With Lumetra, Medicare Program's Quality Improvement Organization

RECOMMENDATION: Approve MOU through October 31, 2008.

11.               Amendments to Agreements With the North Coast Substance Abuse Council (Crossroads) and Alcohol Drug Care Services, Inc. for Providing Substance Abuse Counseling, Non-Medical Detoxification, Residential Treatment, and Other Alcohol and Other Drug Treatment and Recovery Services

RECOMMENDATION: Authorize Chair to sign said amendments.

12.               Amendment to Extend the Agreement With Fortuna Community Services, d.b.a. Humboldt Alcohol Recovery Treatment (HART) for Driving Under the Influence Programs for Fiscal Year 2006-07

RECOMMENDATION: Authorize Chair to execute said amendment, which is required due to the expiration of the previous agreement as well as to clarify the scope of service for each agreement.

Health and Human Services - Public Health Branch

13.               Standard Agreement With the California Department of Health Services for the Immunization Program in Fiscal Year 2006-07 ($78,482)

RECOMMENDATION: Approve Standard Agreement No. 06-55174.

14.               Approve Agreement No. 06-55366, Preventive Health Care for the Aging; Fiscal Year 2006-2009, Fund 1175, Budget Unit 417 ($75,000)

RECOMMENDATION: Authorize Chair to execute agreement with the California Department of Health Services.

Health and Human Services - Social Services Branch

15.               Agreement With Humboldt Domestic Violence Services to Provide Emergency and Supportive Services for CalWORKs' Clients Fleeing Domestic Violence

RECOMMENDATION: Authorize Chair to execute said agreement for $31,211.

16.               Agreement With Humboldt Child Care Council to Provide Services for the Family Preservation Program

RECOMMENDATION: Authorize Chair to execute agreement for $22,500 to provide non-Medi-Cal services through June 30, 2007.

17.               Agreement With Humboldt Child Care Council for Stage One Child Care Services for CalWORKs/WtW Participants for Fiscal Year 2006-07

RECOMMENDATION: Authorize Chair to execute agreement for a maximum reimbursable amount  of $1,133,333.

18.               First Amendment to Agreement With Remi Vista, Inc. for Transitional Housing Placement Program Services to Eligible-Dependent Foster and Probation Youth

RECOMMENDATION: Authorize Chair to execute first amendment to compensate Remi Vista, Inc. at a maximum of $2,747.50 per participant per month, for a total annual compensation not to exceed $197,820.

Public Works

19.               Overflight Easement to the County of Humboldt Executed by Carl and Ellen Reid for the Benefit of the Arcata-Eureka Airport

RECOMMENDATION: Accept Overflight Easement.

20.               Amendment to the Franchise Agreement With Tom's Trash for the Collection and Disposal of Solid Waste in the Willow Creek/Hoopa Area

RECOMMENDATION: Approve amendment to extend said franchise agreement through December 31, 2006.

21.               Courthouse Snack Bar Lease

RECOMMENDATION: Extend lease with Michael Gann, Lessee, through August 31, 2007.

RECOMMENDATION: Adopt Resolution No. 06-__, adopting the proposed budget of the County of Humboldt for Fiscal Year 2006-07 as the final adopted budget for FY 2006-07, including the Board-approved modifications to the proposed budget.  Approve the personnel allocation table corresponding to the budget appropriations.

5.                   Standing Item--Adjourn as County Board of Supervisors, and Reconvene as Humboldt County Public Authority.  Pursuant to Government Code Section 54957.6, Conference with Labor Negotiator.  It is the intention of the Board of the Humboldt County Public Authority to meet in Closed Session to have a conference with its Labor Negotiator to review and instruct designated representatives with respect to the various forms of compensation provided to represented and unrepresented personnel of the Humboldt County Public Authority.

J.                   PUBLIC HEARINGS--1:45 p.m.

Community Development Services

1.                   Close-Out Report for the 2003 Community Development Block Grant Planning & Technical Assistance Grant for Three Projects:  1)  Redevelopment Housing & Business Study; 2)  Living in a Networked World Study; and 3)  Section 504 Evaluation

RECOMMENDATION: Receive staff report and final reports regarding Grant No. 03-PTAA-0041.  Open the public hearing, and receive public testimony.  Close the public hearing.  Direct staff to submit final closing documents, and draw the final ten percent (10%) holdback funds.

County Counsel

2.                   Ordinance Repealing Humboldt County Code Section 2161-1, Division 16, Chapter 1, Title II, Requiring a Real Estate Disclosure Statement for Transfers of Land Subject to the Williamson Act; and Re-Enacting it With an Identically-Numbered Section in Order to Cure a Procedural Error in the First Adoption

RECOMMENDATION: Review and consider the staff report recommending the addition of a new Williamson Act Real Estate Disclosure Statement Ordinance to replace the previous one adopted through a procedural error.  Repeal the previous Williamson Act Disclosure Ordinance No. 2356, adopted on January 3, 2006; and direct that it be deleted from the Humboldt County Code.  Review and consider the new proposed Ordinance No. ____, adding a real estate disclosure statement requirement to the Humboldt County Code for certain transfers/sales of real property subject to the Williamson Act, including the new subsections on public policy and authority; and direct that the new proposed ordinance have the same section numbers as the previously-adopted, but repealed, ordinance.  Introduce the ordinance by title, waive the first reading, and set for adoption on July 11, 2006.
