RECOMMENDATION: Adopt the Resolution.

2.                   Proclaim September 16, 2006 as California Coastal Clean-Up Day (Supervisor Woolley)

RECOMMENDATION: Adopt the Proclamation.

3.                   Resolution Congratulating Humboldt Bay Municipal Water District on its 50th Anniversary (Supervisor Neely)

RECOMMENDATION: Adopt the Resolution.

4.                   Letter of Support for Finding an Appropriate Funding Source for the Reauthorization of the Secure Rural Schools and Community Self Determination Act (HR 2389) (Supervisor Woolley)

RECOMMENDATION: Authorize Chair to sign letter of support.

5.                   Letter to Governor Schwarzenegger in Support of SB 1288 - Expansion of Alcohol and Drug Treatment Benefits for Adolescents. (Supervisor Woolley)

RECOMMENDATION: Approve ratification of letter of support to Governor Schwarzenegger that was signed by the Chair on September 5, 2006.

County Administrative Office (CAO)

6.                   Renewal of Agreement with the California Employment Development Department in Order to Secure Wage and Employer Information for the Purpose of Collection Restitution, Fines, Penalties, Assessments, and Fees Owed to the County or State as Directed by State Mandate or Court Order

RECOMMENDATION: Authorize the CAO or her designee to sign Standard Agreement No. M761811.

7.                   Resolution Amending Resolution No. 82-170 and Rescinding Resolution No. 06-75 - Changes in Office Hours (Eureka Main Library)

RECOMMENDATION: Adopt Resolution No. 06-__.

Health and Human Services - Administration

8.                   Request for Approval of the Compliance Certificates to Allow the Veteran Services Office to Participate in the Medi-Cal Cost-Avoidance Program and the Subvention Program

10.               Temporary Salary Increases for Charlene Pellatz and Justin Sanders

RECOMMENDATION: Authorize a temporary increase in compensation for Charlene Pellatz, Program Coordinator (20 salary ranges), and authorize a temporary increase in compensation for Justin Sanders, Public Health Microbiologist II (20 salary ranges).  (Effective September 10, 2006 until the Public Health Lab Manager position is filled.)

11.               Ambulance Permits

RECOMMENDATION: Renew existing ambulance permits for:  Arcata-Mad River Ambulance Service; Hoopa Health Association; City Ambulance; Fortuna/Garberville Rescue Service; City Ambulance Garberville; and Southern Trinity Health Services.

12.               Agreement with the California Department of Health Services for $25,000 to Fund the Beach Act Monitoring and Notification Program

RECOMMENDATION: Authorize Chair to sign said agreement.

13.               Approval of Subcontract No. 1011422 with the Public Health Institute for Project Leaders Encouraging Activity and Nutrition

RECOMMENDATION: Authorize Chair to sign said subcontract.

14.               Request for Acceptance of a Base Award ($4,131) with the California Department of Health Services for Local Assistance Funding and a Food, Shelter, Incentives, and Enablers Allotment for Tuberculosis Prevention and Control

RECOMMENDATION: Approve request.

15.               Ordinance Authorizing an Amendment to Humboldt County’s Contract with the California Public Employees' Retirement System to Establish a Fire Category for Safety Retirement Coverage

16.               Mud Creek License Agreement with Dale Wyatt

RECOMMENDATION: Approve said agreement for the extraction, crushing, removal, and stockpiling of aggregate in such quantities necessary for County road and other relate road maintenance activities.

17.               Lease Termination Agreement and Lease Agreement for Operation of the Courthouse Snack Bar

RECOMMENDATION: Approve the Lease Termination Agreement with Michael Gann.  Approve the Lease Agreement with Tamara Wright and Eric Bywater through September 30, 2007.

18.               Petition to Form Permanent Road Division (PRD) No. 1000, Zone 003,Sutter Ridge Subdivision, McKinleyville

RECOMMENDATION: Adopt Resolution No. 06-__, Intent to Establish Permanent Road Division No. 1000, Zone 003 and to Impose Parcel Charges for Road Maintenance.  Hold public hearing on November 14, 2006 at 1:45 p.m. to receive testimony of protest regarding the formation and setting of an assessment for road maintenance for the proposed PRD.  Approve supplemental budget of $1,747.

Sheriff

19.               Agreement with Friends for Life Animal Rescue, Inc. for Collaborative Programming That Will Benefit the Animals Served by Humboldt County Animal Services

RECOMMENDATION: Authorize Chair to sign agreement.

Treasurer-Tax Collector

20.               Agreements with Arcata Fire Protection District, McKinleyville Community Services District, Willow Creek Fire Protection District, City of Arcata, Fieldbrook-Glendale Community Services District, Samoa Peninsula Fire Protection District, and Loleta Fire Protection District

RECOMMENDATION: Authorize Chair to sign said agreements for assessment-related fees.  Authorize Chair to sign any similar subsequent agreements submitted by local agencies for collection of assessments.

D.                  SPECIAL PRESENTATIONS

1.                   Resolution of Commendation for Kirk Gothier on His Retirement From the Community Development Services Department (Supervisor Smith)

RECOMMENDATION: Adopt the Resolution.

RECOMMENDATION: Adjourn as the Board of Supervisors.  Convene as the Board of Directors for the Garberville Lighting District.  Authorize Chair to sign said agreement.  Adjourn as the Board of Directors for the Garberville Lighting District.  Reconvene as the Board of Supervisors.

2.                   Agreement for Collection of Special Taxes, Fees, and Assessments

RECOMMENDATION: Adjourn as the Board of Supervisors.  Convene as the Board of Directors for the Hydesville Lighting District.  Authorize Chair to sign said agreement.  Adjourn as the Board of Directors for the Hydesville Lighting District.  Reconvene as the Board of Supervisors.

3.                   Agreement for Collection of Special Taxes, Fees, and Assessments

RECOMMENDATION: Adjourn as the Board of Supervisors.  Convene as the Board of Directors for the Loleta Lighting District.  Authorize Chair to sign said agreement.  Adjourn as the Board of Directors for the Loleta Lighting District.  Reconvene as the Board of Supervisors.

10.               Resolution Authorizing Humboldt County to be a Contributing Signator to the Memorandum of Understanding for a Coordinated Approach to Restoring Fish Passage for Anadromous Salmonids in Coastal Watersheds Through Creation of the Fish Passage Forum; and Authorizing the Chair of the Five Counties Salmonid Conservation Program (5C) to Sign Said MOU on Behalf of Humboldt County 5C

RECOMMENDATION: Adopt Resolution No. 06-__.

Community Development Services

11.               Request for Review and Approval of the Headwaters Fund Board's Recommended Community Investment Fund Grant of Up to $200,000 to the Humboldt Bay Harbor, Recreation and Conservation District for Redwood Dock Modernization Planning

RECOMMENDATION: Approve request, and authorize Chair to sign the grant contract when completed (after review and approval by County Counsel and the Risk Management Director/Deputy CAO).

5.                   Standing Item--Adjourn as County Board of Supervisors, and Reconvene as Humboldt County Public Authority.  Pursuant to Government Code Section 54957.6, Conference with Labor Negotiator.  It is the intention of the Board of the Humboldt County Public Authority to meet in Closed Session to have a conference with its Labor Negotiator to review and instruct designated representatives with respect to the various forms of compensation provided to represented and unrepresented personnel of the Humboldt County Public Authority.
