Board of Supervisors

1.                   Resolution Recognizing Sergeant Robert A. Parton on His Retirement   (Supervisor Geist)

RECOMMENDATION: Adopt the Resolution.

2.                   Letter of Support for Edgar B. Duggan to Continue as a Member of the Trinity Adaptive Management Working Group   (Supervisor Geist)

RECOMMENDATION: Authorize Chair to sign letter of support.

3.                   Resolution of Commendation for John Falkenstrom, Agricultural Commissioner, on His Retirement   (Supervisor Geist)

RECOMMENDATION: Adopt the Resolution.

Clerk of the Board

4.                   Board of Directors--Petrolia Cemetery District

RECOMMENDATION: Accept the resignation of Ben Walker and Kalin Flournoy as Board members.  Approve the appointment of Sterling Mc Whorter and Joe Zanone to fill the remaining terms.

5.                   Approval of the Board Orders from the Meeting of March 30, 2004

RECOMMENDATION: Approve the Board Orders.

Community Development Services--Economic Development

6.                   Grant to the Humboldt Botanical Gardens Foundation Under the Headwaters Grant Fund Program ($40,000)

RECOMMENDATION: Approve grant contract with the Humboldt Botanical Gardens Foundation.

County Administrative Office

7.                   Grand Jury Funding

RECOMMENDATION: Deny the Grand Jury's request for a supplement budget to provide for publication of the annual report ($3,450), additional copying costs and tapes for recording witness testimonies ($1,000), and mileage reimbursement for field investigations ($1,410).  Direct County Counsel to return to the Board with an ordinance to repeal the current ordinance providing for a higher Grand Juror fee than set forth in the Penal Code, effective with the next impaneled Grand Jury.

8.                   Exemptions to the Hiring Freeze

RECOMMENDATION: That the Board approve the exemption for Child Support Services, (1) Child Support Assistant I/II; and H&HS-Social Services, (1) Administrative Analyst I/II.

District Attorney

9.                   Supplemental Budget to Purchase Equipment and Services for Phase One of the Implementation of the DA-STAR Case-Tracking System

RECOMMENDATION: Approve a fund transfer and supplemental budget ($53,208), and direct the Auditor-Controller to establish a Computer Systems line item.

General Services

10.               Ordinance Regulating the Occupancy and Use of Public Rights of Way by Cable Systems and Open Video Systems, Providing for Establishment of Customer Service Standards; Establishing Franchise and Licensing Requirements for Operators of Such Systems and Prescribing Minimum Charges, Terms, and Conditions for and Upon the Construction, Maintenance, and Repair of Such Systems by Adding Division 5 to Title VIII of the Humboldt County Code

RECOMMENDATION: Adopt Ordinance No. ____.

Health and Human Services - Administration

11.               Extension of Extra-Help Hours for Budget Unit 597

RECOMMENDATION: Extend extra-help hours for Nicholas Scott, Department Information Systems Technician (not to exceed 1840 hours).

12.               Allocation of Three 1.0 FTE Payroll/Personnel Specialist Positions in Budget Unit 516

RECOMMENDATION: Approve allocation, exempt new positions from hiring freeze, and direct the Department to return to the Board to disallocate three positions following filling of the newly-allocated positions.

Health and Human Services - Mental Health Branch

13.               Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve the extra-help extension for Kim Evans, Medical Office Assistant I (not to exceed 1440 hours).

14.               Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve extension of extra-help hours for Vanessa Viggers, Medical Office Assistant I (not to exceed 1920 hours).

15.               Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve extension of extra-help hours for Kelly Dunn, Medical Office Assistant I (not to exceed 1300 hours).

16.               Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve extension of extra-help hours for Robert Barkley, Mental Health Maintenance Custodian, (not to exceed 1690 hours), and Jerry Cross, Mental Health Maintenance Custodian, to a maximum of 1725 hours.

17.               Extension of Extra-Help Hours for Budget Unit 424

RECOMMENDATION: Approve extension of extra-help hours for Patricia Garrett, Medical Office Assistant I (not to exceed 1440 hours).

Health and Human Services - Public Health Branch

18.               Extension of Extra-Help Hours for Environmental Health Specialist

RECOMMENDATION: Ratify approval of extension of extra-help hours for Teresa Fregosa to a maximum of 1,825 hours.

Library Services

19.               Total of $6,436 in Donations from Individuals and Organizations to be Spent on Library Materials and a Small Fixed Asset for the Blue Lake Library

RECOMMENDATION: Adopt supplemental budget ($6,436) for receiving donations.

Public Works

20.               Approval of Contract for Seven Quarterly Groundwater Monitoring Events, Waste Profiling and Disposal, and Corrective Action Planning at the Public Works Loleta Maintenance Yard--75 Hookton Cemetery Road, Loleta

RECOMMENDATION: Direct the Director of Public Works to execute the contract with Winzler and Kelly Consultants, and adopt associated budget ($44,369 in reimbursable funds).

21.               Humboldt County Courthouse Remodel and Renovation; Project No. 219000

RECOMMENDATION: Authorize the Public Works Director to sign Change Order No. 7.0 for an additional $50,189 and an additional 144 calendar days.

22.               JLF Construction Final Map Subdivision (Bates Lane); APN 510-111-57, McKinleyville Area

RECOMMENDATION: Authorize the release of Security for Developer's guarantee of work and the release of security for payment of County costs and expenses in the amount of $2,514.

Treasurer-Tax Collector

23.               City of Arcata Redevelopment Bonds of 2004 (Refunding)

RECOMMENDATION: That the Board of Supervisors authorize the Chair to sign the attached "confirmation and consent of County of Humboldt," allowing the City of Arcata Joint Powers Financing Authority to recognize the County's 1988 and 1994 Agreements (collectively, the "Agreement") with the Community Development Agency of the City of Arcata (the "Agency") and agreeing to have the County subordinate its tax increments pass-through property taxes and allowing the City of Arcata to amend its Redevelopment Plan to increase the effectiveness of the plan for one additional year because certain conditions have been met.

RECOMMENDATION: Adopt Resolution No. 04-__, making the required findings for certifying compliance with the California Environmental Quality Act; and approving the PWM, Inc. modification to Case No. SP-01-26M, subject to the revised conditions of approval as set forth in the submitted Attachment F.
