3.                   Agreement for Consulting Services for Third Party Review of the Samoa Town Master Plan Tsunami Vulnerability Report and Supplemental Budget (277)for Planning Division

RECOMMENDATION: Execute the agreement for preparation of a Special Services Third Party Review; Authorize a trust fund account to be used as an escrow account; Authorize increase in Professional Services Expense Account # 1100-277-2-118 with costs to be offset by account # 110-277-608-061

4.                   Ray Roeder Joint Timber Management Plan and Guide

RECOMMENDATION: Approve the division of assessor parcels zoned Timberland Production Zone and the Ray Roeder Joint Timber Management Plan and Guide.

Health and Human Services - Mental Health Branch

5.                   Proposal for a Mental Health Wellness Center

RECOMMENDATION: Authorize staff to negotiate with Redwood Community Action Agency (RCAA) for the creation of Mental Health Wellness Center as described in the Request for Proposal approved by the Board of Supervisors on July 11, 2006.

Health and Human Services - Public Health Branch

6.                   Extension of Extra Help Hours for Paul Mollard

RECOMMENDATION: Approve the extension of extra-help hours for Paul Mollard, Community Health Outreach Worker, to a maximum of 1800 hours during fiscal year 2006-2007 in Budget Unit 414, Health Education, pursuant to Section 7 of the Humboldt County Salary Resolution effective immediately.

7.                   Extension of Extra-Help Hours for Maria Garcia

RECOMMENDATION: Approve the Extension of Extra-help Hours for Maria Garcia, Office Assistant I to a maximum of 1920 Hours during the fiscal year 2006-2007 in Fund Budget 1175, Budget 400.  There will be no impact on the General Fund.

Health and Human Services - Social Services Branch

8.                   Second Amendments for Three Separate Leases of the Modular's located at  628 West Clark Street.

RECOMMENDATION: Approve the three Second Amendments extending the leases executed April 25, 2000 with Johnson's Mobile Solutions

9.                   Department Plan for the Provision of Transitional Housing Program

RECOMMENDATION: Authorize the Director of the Health and Human Services Department - Social Services Branch to submit the County's Transitional Housing Program - Plus Plan to the California Department of Social Services for their approval.   There will be no impact to the General Fund.

Probation

10.               Bar-O-Boy's Ranch Contract

RECOMMENDATION: Approve and execute the attached contract between the County of Del Norte and County of Humboldt to allow for the utilization of the Bar-O-Boy's Ranch on an as-needed, space available basis. There is no anticipated impact to the General Fund.

Public Works

11.               Retroactive Extension of Extra Help Hours for John Dockham

RECOMMENDATION: Retroactively approve the extension of extra help hours for John Dockham by 94 hours to a maximum of 1054 hours each for Fiscal Year 2006/2007, pursuant to Section 7 of the Humboldt County Salary Resolution.  No impact to the General Fund.

12.               Extension of Extra Help Hours for Randy Ganoung

RECOMMENDATION: Approval of extending Randy Ganoung's extra-help employment by 960 hours to a maximum of 1920 hours for fiscal year 2006-2007 pursuant to Section 7 of the Humboldt County Salary Resolution. (No General Fund Impact)

13.               License Agreement for Advertising Display Unit with Cornucopia Enterprises at Arcata/Eureka Airport

RECOMMENDATION: Approve and authorize the chairperson to execute the attached License Agreement, in duplicate with Cornucopia Enterprises.

14.               Timber Fall LLC Final Map Subdivision, APN 17-015-33, Eureka Area

RECOMMENDATION: Approve and accept the subdivision map for recordation; Accept the dedications to the County of Humboldt of Timber Fall Lane, Timber Fall Court and Parcel A for public utilities.

RECOMMENDATION: Receive and File the form of the Statement of Annual Financial Information and Operating Data of the County of Humboldt, California, "Annual Report".
